Creating a Sense of Home: We are home to 108 residents. At Windsor Elms Village there are three distinct neighbourhoods. Each neighbourhood has four houses of nine residents. With such a diverse group of residents, you will find each house has its own character. At WEV, we strive to create flexibility that mirrors life in the communities and homes our residents have known.


The Social Worker will help our residents and their families and our staff with mental health and well-being, and help them navigate the health care system. As key members of an interdisciplinary team, social workers help provide comprehensive care designed especially for residents. They also work collaboratively as part of the care team to help residents and families understand goals of care & build an environment that honours the dignity of residents. Every resident is a unique individual with a rich history, and the social worker ensures treatment plans reflect individual needs and strengths. The Social worker honours diversity and different ways of knowing. They build an inclusive and safe environment for residents, families and staff. When needed, social workers advocate for and with residents.


Summary of Roles & Responsibilities (Full Job Description available upon request)
• Support Move in Process: Assist with smooth transitions into care, ensuring residents and families feel informed and supported.
• Counseling and Emotional Support: Provide compassionate guidance to residents, families & staff care partners navigating challenges with individuals or in a support group setting.
• Care Planning and Case Management: Collaborate with the interdisciplinary team to develop and implement individualized care plans that address each resident's unique needs. Contribute in an interdisciplinary team to provide comprehensive care
• Collaboration and Advocacy: Work closely with residents, families, and interdisciplinary teams to advocate for the best outcomes.
• Professional Contributions: Stay current on best practices and contribute to ongoing quality improvements in care delivery.
• Showcase our Home: Provide tours to community members to showcase our home & our Eden Alternative Culture to others.
• Assessments: Conduct psychosocial, cognitive and mental health assessments
• Support the Residents Journey at Windsor Elms Village: Help residents & families adapt to transitions, grief and loss, and other challenges & Liaise with outside agencies to offer additional support or augment what already exists
• Educate: Develop and deliver education to families and staff


Qualifications:
• Registration in good standing with the Nova Scotia College of Social Workers
• Bachelor's degree or master's degree in social work from an accredited university
• Experience in Long Term Care or Dementia is considered an asset
• Values that align with our Eden Alternative Approach to Care


Salary: $75,000 - $85,000 Based on Experience
Hours: The regular hours of work for this position are intended to be 8:30am to 4:30pm Monday through Friday. Flexibility may be required outside of these hours.
Start Date: As soon as possible
