Resident Support Facilitator Permanent Full-time

Join Our Team as a Resident Support Facilitator: Are you passionate about making a difference in people's lives? We're looking for a compassionate and dedicated Resident Support Facilitator to support residents and their families throughout their long-term care journey. In this role, you'll have the opportunity to work flexible hours while playing a meaningful part in improving the well-being of others.

About the Role
As a Resident Support Facilitator, you will report to the Director of Clinical Services and ensure residents' emotional, social, and practical needs are met. Your work will reflect our philosophy of care, focusing on collaboration, advocacy, and support to create a positive and fulfilling environment.

What You'll Do

  • Admissions Coordination: Assist with smooth transitions into care, ensuring residents and families feel informed and supported.
  • Counseling and Emotional Support: Provide compassionate guidance to residents and families navigating challenges.
  • Care Planning and Case Management: Collaborate with the interdisciplinary team to develop and implement individualized care plans that address each resident's unique needs.
  • Collaboration and Advocacy: Work closely with residents, families, and interdisciplinary teams to advocate for the best outcomes.
  • Professional Contributions: Stay current on best practices and contribute to ongoing quality improvements in care delivery.

What We're Looking For

  • Formal training in counseling, social work, psychology, or a related field. A degree in social work is an asset but not required.
  • At least 3 years of experience in long-term care, geriatrics, or community support settings.
  • Exceptional interpersonal, communication, and organizational skills.
  • A commitment to supporting residents with dignity, compassion, and respect.

Hours of Work: Non-union Regular Full-time (1.0 FTE) - Typically 40 hrs per week Monday - Friday (hours may flex as necessary) Paid 80 hrs bi weekly, paid breaks and lunch.
Salary: Annual salary (based on experience according to Professional Social Worker Scale for LTC)
Benefits: Health & Dental and Optional' s, Pension Plan (Manulife/NSEHPP (HANS), Corp rate for Gym
