We are seeking a knowledgeable and compassionate Systems Navigator/Social Worker to join our team. This role is essential to supporting our residents in accessing and navigating the complex network of health and social services in Nova Scotia.
Position Summary
Reporting to the Chief Administrative Officer, the Systems Navigator/Social Worker will play a critical role in enhancing the lives of residents by connecting them to appropriate health, social, and provincial programs. As a key member of the interdisciplinary allied health team, the successful candidate will leverage their extensive knowledge of Nova Scotia's healthcare and long-term care systems to advocate for residents, provide system navigation expertise, and ensure seamless access to supports that promote their goals and well-being.
Key Responsibilities
•Support residents and their families in navigating the health and social services systems in Nova Scotia, including long-term care, healthcare programs, and community supports.
•Collaborate with residents to identify and address their individual needs, goals, and barriers to accessing services.
•Provide expert guidance on provincial programming and supports to assist residents in achieving their personal and healthcare goals.
•Act as a liaison between ECL residents, families, healthcare providers, and external agencies to ensure effective communication and coordination of services.
•Advocate for residents' rights and needs within the healthcare and social services systems.
•Stay informed about current provincial policies, programs, and initiatives related to long-term care and healthcare in Nova Scotia.
•Assist ECL leadership in understanding and implementing provincial programming to benefit residents.
•Participate in interdisciplinary team meetings, contributing expertise to resident care planning.
•Lead multidisciplinary care planning conferences with residents and family.
Qualifications
•Bachelor's degree in social work, Social Sciences, or a related field.
•Extensive experience navigating Nova Scotia's healthcare system or long-term care sector.
•Demonstrated ability to advocate effectively for individuals within complex systems.
•Strong understanding of provincial programming, supports, and resources relevant to long-term care.
•Exceptional communication, interpersonal, and organizational skills.
•Ability to work collaboratively as part of an interdisciplinary team.
•Empathy, compassion, and a commitment to resident-centered care.
