We seek a compassionate individual for the Clinical and InterRai Neighborhood Manager role, reporting to the Director of Care. This role oversees daily resident care operations, fosters clinical excellence, and leads neighborhood teams. The RAI Coordinator reviews resident assessments, manages documentation, and supports the nursing and interdisciplinary team. This leadership role ensures regulatory compliance, mentors staff, enhances clinical processes, and aligns with The Meadows' mission and values to enrich residents lives daily. The individual must also be willing to take on various projects and duties as needed.

WHAT YOU'LL DO

Inter-RAI & Admissions Management:

· Providing ongoing leadership and mentoring for education, data accuracy, data submission and staff compliance and competencies of interRai

· In collaboration with the multidisciplinary team, responsible for leading, coordinating and implementing best practices in the Resident Assessment Instrument-Minimum data Set (RAI-MDS) program, providing support for accurate RAI applications, assessments, audits, monitoring quality indicators, completion of data submission process and staff education, coaching and compliance.

· Oversee RAI assessments and oversee flow of residents (admissions, discharges, transfers)

· Manage coordination and timely completion of Resident assessments, tracking forms, Resident Assessment Protocols and Resident Care Plans.

· Chairs Care Plan Meetings

· Participate in weekend/on call rotation to ensure new admissions coverage

Resident Care Management:

· Ensure residents' physical, social, emotional, and spiritual needs are met by the team

· Works collaboratively with all neighborhood members, departments and interdisciplinary teams.

· Work with interdisciplinary teams to enrich residents' lives

· Be an escalation point for resident and family concerns

· Foster communication between residents, families, and healthcare professionals.

Leadership & Staff Management:

· Provide mentorship, training, and support to nursing and neighborhood team.

· Assist with recruitment and performance and neighborhood goal setting

· Train and develop new staff, referring concerns to the DOC/ADOC/Department Managers.

· Provide support and needs for staff on best practices in resident care and resident-centered care.

· Promote continuous learning and professional growth among the care team

· Lead huddles, problem-solving and team meetings

· Cover for other roles during absences (i.e. ADOC, Care Navigator, Infection Control)

Compliance & Quality Assurance:

· Ensure adherence to licensing requirements

· Conduct audits and quality improvement reviews.

· Support leadership in policy implementation and workforce planning.

· Works in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act/Regulations.

WHAT YOU NEED TO HAVE

· Registered Nurse in good standing with the College of Registered Nurses of Nova Scotia.

· 3 years leadership/management experience

· Proven leadership experience and a commitment to continuous learning.

· Willingness to upgrade and maintain qualifications as required by evolving job demands.

· Genuine passion for engaging with residents, enhancing their social, emotional, physical, and spiritual needs.

· Ability to work harmoniously within a multidisciplinary team, lead and support the Neighborhood Team, and uphold the organization's values and philosophy.

· Adherence to professional competencies, including caring and respectful behavior, therapeutic relationships, strong clinical and assessment skills, and communication skills

· Self-directed, resilient to change, and capable of facilitating, resolving conflicts, and leading projects and programs. While maintaining excellent organizational, analytical, and time management skills.

· Reliability and availability to attend work.

· Well-organized, able to initiate workload independently, while collaborating closely with the Neighborhood Team, Director of Care, managers, and other departments.

· Commitment to promoting and supporting a positive work environment while maintaining strict confidentiality.

· Excellence in computer skills, including experience with Inter-Rai /Civica/Momentum

· Agreement to have the role revised and restructured based on operational requirements.

· Ability to manage conflicts effectively and build teams

· Commitment to employment equity, diversity, and inclusion in the workplace.

WHAT WE'D LOVE YOU TO HAVE

· Long Term Care experience

· Possession of valid CPR, and First Aid Certificates.

· Leadership courses (Team building Training / People Centered Training)

· Quality Systems methodology training and experience (i.e lean)

· Familiar working in a unionized environment
