Horizon Health Network in beautiful New Brunswick is looking for a Primary Health Care Standardization Coordinator to join our team!

Horizon Health Network (Horizon) is seeking a committed, strategic, and detail-oriented individual to join our team as a Primary Health Care Standardization Coordinator. The ideal candidate will have experience in primary health care, policy and standards development, quality improvement & process improvement methodology and will possess strong analytical, communication, and problem-solving skills to drive positive outcomes for the primary health care program.

Who you are

  • Bachelor's degree in health sciences or related field, or related experience would be an asset
  • Education related to quality assurance/quality improvement and policy development a definite asset
  • Demonstrated experience in executing quality assurance/quality improvement programs and activities
  • Excellent analytical skills, including the ability to think outside existing management practice/models and demonstrate foresight about potential issues and opportunities
  • Minimum five years of experience in a community health care setting, preferably in Primary Health Care
  • Demonstrated ability to engage others in discussions and influence decisions through clear, concise, logical, and compelling communication while providing the appropriate level of depth when communicating with a diverse group
  • Aptitude for delivering an intended message in a manner reflective of a caring and professional organization
  • Effective communication skills with individuals at all levels of the organization, and external partners
  • Understanding of primary health care principles and primary health care delivery systems and inherent safety, risk management and quality processes
  • Knowledge of accreditation standards and requirements, especially in a community environment
  • Proficient in the use of computers and information systems, including Microsoft Office applications
  • Ability to write clear and succinct reports
  • Ability to maintain strict confidentiality
  • Ability to gather and analyze data and formulate comprehensive reports
  • Ability to consider multiple aspects of information and the impact on the patient, families, public and the organization
  • Ability to attend work on a regular basis
  • Ability to perform the duties of the position
  • Good work record
  • Written and spoken competence in English is required

What you will do

This position is responsible for regional planning, implementing, and monitoring strategies for quality components of the clinical care delivery in Horizon Primary Health Care program, with the goal of enhanced standardization of clinical processes across Horizon Primary Health Care sites.

The Primary Health Care Standardization Coordinator is required to work in collaboration with all areas of the organization, our patients, and their families, as well as with external stakeholders in the development of strategies and their operational implementation. This includes physicians, nurse practitioners, as well as all members of the primary health multidisciplinary team.

This role will support and make recommendations to the Primary Health Care Leadership team, regarding clinical quality initiatives with a focus on patient care, medical directives, policies, accreditation, and other performance standards. They will communicate with employees, patients, their family, other healthcare providers and stakeholders as required to accomplish his/her functions.

  • Responsible to identify and maintain a clinical quality assurance/quality improvement structure and process for Primary Health Care program
  • Ensure Accreditation Canada requirements are met for Primary Health Care program, and establish a plan to manage ongoing accreditation obligations
  • Develop and maintain a standardization work plan, including regional policies & standards
  • Support the Horizon Quality and Safety structure and processes for Primary Health Care
  • Conduct research and best practice reviews, develop plans to implement best practices
  • Support the Primary Health Care team in the development and maintenance of quality, safety and risk indicators and reporting process
  • Ensure the utilization of reporting measures that are relevant, measurable and provide data to drive performance initiatives within the organization
  • Participate or lead relevant committees
