Job Summary
The Housing Support Liaison Worker plays a crucial role in providing direct assistance and advocacy for individuals living with severe and persistent mental illness and concurrent disorders who are experiencing housing instability or homelessness and/or having difficulty navigating social support systems. Reporting to the FACT Manager and working as part of a multidisciplinary team, the incumbent will:
Educate individuals about available housing options, services and supports; tenant and landlord rights; and eviction prevention skills, such as basic household cleaning and upkeep;
Assess housing needs, develop individual support plans, and advocate on behalf of individuals to find and maintain housing;
Assist individuals with housing and financial assistance applications, referrals and related processes;
Perform independent work in the community to actively seek and maintain a database of prospective housing options;
Consult and build partnerships with local landlords, community agencies and other service providers, such as homeless shelters and local food banks, to coordinate services and resources for individuals;
Support individuals in fostering positive relationships with landlords, homeless shelter staff and co-occupants by building coping, emotion regulation and conflict resolution skills;
Provide intervention, mediate issues and support landlords and individuals facing eviction;
Liaise with other departments and program areas regarding discharge planning and supports needed to facilitate discharge and transition to various stepped-down supportive housing options; and
Complete and maintain necessary records and documentation.

Visual or auditory concentration to observe and listen to individuals in need of support;

Travel within applicable health zone; and
The use of a private vehicle.
While primary responsibility is with the FACT team, incumbent may also be required to support individuals from other areas of Mental Health and Addictions Program.
Job Qualifications
Successful completion of a bachelor's degree in behavioral or social sciences is required.
Minimum of three years' experience within the last five years working with adults diagnosed and living with severe mental illness is required.
Knowledge of current community mental health assessment and intervention techniques.
Knowledge of mental illness from bio-psychosocial perspectives, and holistic and community approaches.
Proven ability to provide counseling and education to mental health clients and their families.
Demonstrated ability to work collaboratively in a multi-disciplinary team environment and maintain positive working relationships.
Use of a private vehicle is a condition of employment.
A satisfactory record of work performance and attendance is required.
