Definition of Work:Youth Case Management is a service designed to ensure that youth with addictions and complex mental health needs receive an appropriate level of care, treatment, and support. The Mental Health and Additions Youth Case Manager is a specialized position within Mental Health and Addictions Services, providing assessment and case management services to youth between the ages of 12 – 18 within the Central Region. This position works closely with staff of provincial treatment centers to ensure effective discharge planning and after care support services. This position re-ports to the Regional Manager of Mental Health and Addictions. The successful candidate will be committed to safety and partner with all team members to im-prove worker and patient safety by integrating safety into work practices, identifying risks, and implementing improvements.
Duties:
• Functions with a high level of autonomy while functioning as a member of interdisciplinary teams.
• Uses evidence based practices to plan, implement, and evaluate treatment modalities for youth with complex mental health and additions needs.
• Assesses, develops, and implements interventions regarding mental health and addictions issues for delivery to youth, and their families.
• Advocates on behalf of individuals with schools, service providers, and the community.
• Liaises between the youth and other individuals and services involved with the youth.
• Works in partnership with community youth serving agencies throughout the province to promote the development of a continuum of services in the community to meet the needs of youth as they transition from the treatment centre(s) to their home communities.
• Participates in case consultation and case coordination.
• Offers Doorways Services as required.
Required qualifications:
• Master’s Degree in Social Work.
• Five (5) years current experience in a mental health and/or addictions setting.
• Current registration with the NLASW.
• Demonstrated knowledge of adolescent development and current best practice approaches used to treat youth with concurrent disorders.
• Willing to work a flexible work schedule.
• A satisfactory Child, Youth, and Family Services Records Check.
• Use of a private vehicle and valid driver’s license is a condition of employment.

• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory certificate of conduct from the RCMP or local police authority.
