Job Summary

The Licensed Practical Nurse will work within an inter-professional, collaborative team environment to provide access and continuity of care to improve health outcomes and patient quality of life, with the support and wrap around services of the Family Care Team. The Licensed Practical Nurse will practice within their full scope, providing quality nursing care, including:

  • Comprehensive assessment of persons and families of all ages, incorporating the Determinants of Health and using the nursing process to plan, implement and evaluate individualized plans of care.
  • Supporting people and their families to achieve and maintain optimal health and well-being through education, health coaching, motivational interviewing, consultation and collaboration
  • Utilizing a Person and Family Centered approach, identifies personal goals of care to inform a plan of care developed collaboratively with the interprofessional team.
  • Utilizing the BETTER program approach for preventative screening and chronic disease prevention.
  • Supporting the management of chronic disease and Ambulatory Care Sensitive Conditions (ACSC) through collaboration with the interprofessional team.
  • Facilitating integrated Virtual Care options , including remote patient monitoring for chronic disease screening, prevention and management
  • Recognizing cultural diversity and practicing culturally safe care.
  • Practicing age-friendly care in a collaborative care team setting.
  • Participating in clinic evaluations and continuous quality improvement processes based upon current best practice guidelines.

Required Qualifications:

  • Graduation from an approved Practical Nursing program
  • Current registration with the College of Licensed Practical Nurses of Newfoundland and Labrador (CLPNNL)
  • A minimum of 5 years of experience within the last 7 years working in an adult acute care medicine/surgical setting, primary health care setting, or community clinic setting
  • Current BLS certification
  • Demonstrated effective oral and written communication skills.
  • Demonstrated strong collaboration skills and the ability to create and maintain successful working relationships.
  • Ability to self-direct and work independently and collaboratively with team members
  • Proficiency with TELUS Med Access EMR, Meditech, CRMS and Microsoft Office tools.

Additional Qualifications/ Assets:

  • Successful completion of an approved course in chronic disease prevention and management (BETTER; Improving Health My Way)
  • Knowledge and experience with integrated Virtual Care in primary health care service delivery
  • Knowledge in change leadership and quality improvement methodologies
  • Experience in a collaborative team model
  • Experience working in and understanding of the unique challenges and opportunities of rural health care services delivery
  • Understands systemic racism and applies concepts of cultural safety in engaging with First Nations, Inuit and Metis communities.
