The philosophy of a Family Care Team (FCT) is to deliver care that is patient-centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client’s need. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT’s will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers. The Exploits Family Care Team will include the geographical areas of Grand Falls-Windsor, Bishop’s Falls, Botwood and Buchans. This position will also provide support for the
Coast of Bays Family Care Team. Travel throughout the region will be required.


Definition of Work:

The Registered Nurse IIC (Care Facilitator) provides nursing leadership within the Family Care Team Clinic and collaborates with the Manager to
facilitate the overall operation of the clinical area. The successful individual will be expected to ensure standards of care are appropriate while implementing strategic programming which includes implementing clinical standards, policies, procedures, new workflows, clinical care pathways, and research. The position will also provide clinical expertise and consultation to the multidisciplinary team. Other duties may include assessing educational competencies of staff, planning orientation, chart audits, conflict resolution, participating in departmental meetings and committees, acting as a regional resource for specialized services, complex case management support, developing processes for health navigation and care coordination across program areas, analyzing
program statistical data and direct clinical care. Flexible hours are required (combination of days, evening and weekends required). The Registered Nurse IIC reports to the Regional Primary Health Care Program Manager responsible for the service.


Job Qualifications:

*Graduation from an approved/recognized post-secondary nursing education program
with a Baccalaureate degree in Nursing or equivalent education or training is required.
*Current practicing licensure with the College of Registered Nurses of Newfoundland and
Labrador (CRNNL) is required.
*Minimum of five years of experience within the last seven years working in an
autonomous/independent practice area is required.
*Primary Health Care experience is an asset.
*Current BLS Certification is required.
*Knowledge of continuous quality improvement concepts.
*Demonstrated competency in organizational, leadership, interpersonal, team-building,
communication, teaching and decision-making skills.
*Demonstrated initiative and ability to work independently.
*Demonstrated progressive leadership.
*Evidence of active involvement or willingness to participate in continuing professional
education development.
*Membership in professional organizations/special interest groups is an asset.
*A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private
vehicle are conditions of employment.
*A satisfactory record of work performance and attendance is required.
*Previous EMR experience is an asset.
