Job Summary
The primary clinical purpose of this position is the provision of advanced professional social work services to clients in the emergency departments and acute care settings at both the HSC and St. Clare's sites. This includes providing clinical leadership, supervision, consultation, monitoring, and support in the planning, development, coordination and evaluation of social work practices within the Clinical Efficiency and for programs and services that participate in and are impacted by patient/client access and flow. As an integral member of the interdisciplinary team the social worker independently screens and assesses clients, formulates treatment and discharge plans and provides follow-up and aftercare. The goal of social work services is to improve health outcomes by providing services to individuals, families and groups that maximize their use of health and community services. In addition to clinical responsibilities, the Social Worker participates in a variety of administrative and community development activities and provides supervision to social work students.

The Social Worker works with a high degree of autonomy and is the sole charge social worker after hours and on weekends. In the program management model, social work intervention requires independence in clinical judgment, critical thinking, and decision making in the delivery of services. Therapeutic intervention involves a variety of counseling methods including individual, family, group and interdisciplinary teamwork.

This position reports to the Regional Director, Clinical Efficiency and maintains accountability to the Professional Practice Consultant for Social Work for professional integrity and practice issues.

Job Demands

Regularly sits to consult with clients, conduct therapeutic sessions, and to work on the computer.
Occasionally works in awkward or cramped body positions; pushes, pulls, and sets up furniture for group work, and drives to conduct home visits with clients.
Regularly uses fine finger/precision work on a computer.

Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador College of Social Workers (NLCSW) is required.
Minimum of three years of experience within the last five working as a Social Worker in an adult acute care or community support program is required.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of discharge planning and community-based resources.
Knowledge of crisis interventions and supportive counseling.
Knowledge of case management.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated strong clinical leadership skills.
Demonstrated ability to work effectively within an interprofessional environment and provide clinical consultation.
Demonstrated ability to work with a high degree of independence and initiative.
Demonstrated strong program development and evaluation skills.
Demonstrated analytical, critical thinking, problem solving, decision-making and organizational skills.
Demonstrated strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
Demonstrated ability to direct, supervise and mentor students.
Demonstrated ability to establish and maintain effective working relationships.
Demonstrated ability to support quality improvement initiatives.
Demonstrated strong awareness of ethical principles and standards of professional conduct and practice.
A satisfactory record of work performance is required.
