Job Summary
The primary responsibilities of this position are to provide services of a practical nature to patients and their families. Services will include procurement of medical equipment/supplies required for discharge, arrangement of transportation, liaison with the Department of Human Resources and Employment regarding financial services and drug cards, and other services as required. The Social Assistance Worker will work in a supportive and assisting capacity to social workers.

Regularly moves/lifts objects less than 10 lbs. and occasionally may provide assistance to lift object up to 30 lbs with push/pull light to medium forces for patient handling.
Constantly sits and performs fine finger precision work when using a computer.
Regularly stands, walks, and drives to client visits.
Alertness and concentration required while frequently completing repetitive tasks and constant auditory/visual concentration.
Job Qualifications
Graduation from a recognized/approved bachelor's degree in Social Sciences/Humanities or Business is required.
Knowledge of financial assessments for home supports, medical discharge, long-term care, assistance and/or subsidy programs.
Knowledge of community support agencies, programs and eligibility requirements.
Knowledge of complex health concerns and/or mental health issues.
Demonstrated strong critical thinking, problem solving and time management skills.
Demonstrated strong advocacy skills.
Demonstrated ability to work independently.
Demonstrated strong interpersonal skills with clients/patients/families and various community resources.
Demonstrated strong oral and written communication skills.
A satisfactory record of work performance is required.
The Mental Health & Addictions Program offers promotion/prevention, urgent/emergent, rehabilitation, and continuing care services to children, youth and adults in acute care, community, and correctional health settings. Services range from assessment, counselling, detoxification, treatment and follow-up to rehabilitation, housing, case management and leisure programming. We are located at various hospital and community sites, in urban and rural settings, throughout the eastern region.
