Job Summary
This Social Assistance Worker is part of an inter-disciplinary team responsible for the monitoring and licensing of Personal Care Homes. Responsibilities of this position will include monitoring activities to ensure compliance of Personal Care Homes with the Provincial Personal Care Home Program Operational Standards and completing related reports for assigned Personal Care Homes. The Social Assistance Worker will participate in team meetings, licensing meetings and investigation of complaints and must have the ability to work independently with limited supervision. They will document in CRMS, and complete monthly statistical reports as required. The successful candidate will provide in-service/education to PCH operators and staff on topics including, but not limited to, documentation, incident reporting and the Adult Protection Act, as well as developing presentations with the interdisciplinary team. This position will also meet with residents to review eligibility for supplementary benefits, ordering medical supplies through special assistance program, completing referrals to other services, review staffing schedules, and perform other related duties as required.

Job Qualifications
Graduation from a recognized/approved Bachelor's Degree in Social Sciences/Humanities or Business is required.
Knowledge of financial assessments for home supports, medical discharge, long-term care, assistance and/or subsidy programs.
Knowledge of community support agencies, programs and eligibility requirements.
Knowledge of complex health concerns and/or mental health issues.
Demonstrated strong critical thinking, problem solving and time management skills.
Demonstrated strong advocacy skills.
Demonstrated ability to work independently.
Demonstrated strong interpersonal skills with clients/patients/families and various community resources.
Demonstrated strong oral and written communication skills.
The use of a private vehicle is a condition of employment.
