Horizon Health Network in beautiful New Brunswick is looking for a Regional Manager Chronic Disease Prevention & Management to join our team!
Horizon Health Network (Horizon) is seeking a candidate that demonstrates the ability to think strategically at a regional at systems level, demonstrates ability to communicate effectively with a wide variety of system stakeholders, from frontline to administration leaders and can work independently and autonomously, and show initiative. Who you are
You have:
- A Bachelor's degree in a health discipline. A Master's degree in a health discipline or health leadership/administration is an asset
- Five years of healthcare experience, with leadership experience
- Experience in program planning, implementation and evaluation in the areas of
- chronic disease prevention & management, population health, and health
- promotion
- Demonstrated competence with quality improvement strategies, such as:
- facilitation, change management, process improvement, project management,
- lean six sigma
- The ability to prioritize, organize workload and collaborate with others
- Experience of managing human resources an asset
- Proficient computer skills with Microsoft (Word, Outlook, Power Point,
- Excel) and QI software, ie: Visio, MIRO
- Physically able to perform the duties of the position
- Able to travel to other areas of the organization
Written and spoken competence in English is required at an advanced level. The Ability to speak French is an asset.
What you will do
- Lead in the development of integrated care pathways and systems for the prevention & management of chronic conditions for PHC program, using quality improvement and process improvement methodologies.
- Lead in the dissemination and integration of Horizon Chronic Disease Standards across Horizon Primary Health Care program, and across the organization with applicable Clinical Networks/Programs/Services.
- Lead in the review and revisions of Horizon Chronic Disease Standards.
- Determine regional PHC priorities related to chronic disease prevention & management based on stakeholder feedback and current evidence and best-practices.
- Manage the regional PHC quality initiatives related to chronic disease & prevention, providing guidance and support with the implementation of the initiatives in each Area.
- Foster and support collaboration with stakeholders (internal and external) for the identification of priorities and improvement of chronic disease prevention & management.
- Provide expert consultation and facilitation for the implementation of chronic disease standards, care pathways, and processes.
- Lead &/or assist in the development of policies and medical directives related to chronic disease prevention & management.
- Conduct research, data collection, and data analysis of various data sources that impact chronic disease prevention & management and population health.
