Horizon Health Network (Horizon) is seeking an engaging, innovative, and intrinsically motivated individual to join our team as an Action and Response Lead for the Community Health Needs Assessment (CHNA) Team. The ideal candidate will have experience engaging, facilitating, and working collaboratively with community partners, internal Horizon staff, and other governmental bodies to develop, monitor, and report on action-based solutions to specific community health needs identified in the CHNA.

Who you are
You have a worked in the healthcare industry or other related fields, with preference given to experience in positions involving staff and/or community engagement, developing strategic relationships to achieve quantifiable, tangible, and recordable results for a minimum of 3-5 years. You are skilled in building and maintaining relationships with key internal and external stakeholders while addressing community needs with competing priorities. You have a high working proficiency in Microsoft Office applications such as PowerPoint, Excel, Word, Outlook, etc. You also have strong writing and knowledge translation skills as reporting writing and sharing of information are core functions of this position. Experience with process and/or program evaluation is an asset.
Written and spoken competence in English is required at an advanced level. Written and spoken competence in French is an asset.
What you will do
Job Purpose:
Reporting directly to the Clinical Executive Director of Community Health, the CHNA Action and Response Lead will work as a part of the CHNA team within the Community Health Department. The CHNA team consists of a CHNA coordinator, two CHNA Leads (one with a primary focus on research and engagement and this role focused on action and response), community developers, as well as other selected Horizon personnel. The selected candidate will co-lead the planning and facilitation of Horizon's CHNA process which includes supporting the coordination of the CHNA communication, strategy, leading action, process facilitation, and providing a written response report and infographics back to the community to communicate how CHNA health needs will be addressed in their communities. The role will also support, as needed, the coordination and planning for CHNA's, data gathering, and evaluation aspects of the CHNA process.
Qualifications:
- Master's degree in health care, community health, population health, public health, or a related field of study. A PhD is considered an asset. A combination of education and work experience will also be considered;
- Experience with community and right-holder/ stakeholder engagement;
- Experience with interpreting, and integrating qualitative data to support the identification of population health needs and to support health care decision making an asset;
- Ability to interpret secondary quantitative data sources;
- Ability to engage and work collaboratively with community partners, other CHNA stakeholders, and health sector partners;
- Strong group facilitation, knowledge translation and report writing skills;
- Strong understanding of population health, health care equity, community development and the social determinants of health;
- Database and process management experience;
- Ability to delegate tasks to CHNA Coordinator or administrative support as appropriate;
- Ability to function independently with initiative and creativity.
Flexibility to work with modified work schedules is needed (some weekends and evenings may be required)
A valid driver's license and ability to travel within the province is required.
Key Responsibilities:
- Develop and maintain internal CHNA action and response reporting, monitoring, communication, and evaluation processes.
- Develop a plan to engage internal and external stakeholders towards actioning identified health needs.
- Plan and facilitate engagement and communication activities for internal and external partners.
- Review, incorporate, and communicate currently available data to support action planning and decision making.
- Coordinate the sharing of CHNA reports with applicable Horizon, Government, and Community organizations.
- Facilitate and coordinate Horizon's ongoing communication with community regarding the work in progress related to the identified population health needs.
- Build and maintain important internal relationships with those accountable for identified CHNA needs.
- Work collaboratively with CHNA team and Community Heath team as well as independently
