The New Brunswick Extra-Mural Program (EMP), often known as the "hospital without walls," has operated under the simple philosophy of helping New Brunswickers receive the care they need in the places where they are healthiest and happiest - their homes and communities. JOB SUMMARY

To holistically coordinate the care of each patient, incorporating the nursing profession's values of knowledge, theory and practice. To assess, plan, implement and evaluate the needs of patients and significant others in the delivery of care and collaborate with all members of the interdisciplinary team in this approach.

The EMP Enhanced Care Coordination in Schools (CCS) program will provide support to Education and Early Childhood Development (EECD) and integrated care teams in all regions throughout the province. This includes the care coordination required to implement each student's school-based medical care plan while continuing to evaluate the needs of students/families and care providers (including school personnel) in the delivery of care. The EMP Coordinator works to facilitate and coordinate activities required for the enhancement of care for students with complex medical care needs.

KEY RESPONSIBILITIES

  • Work with students, families, teachers, educational assistants, other professional staff to ensure an integrated person-centered approach to meet the student's care needs in a time efficient manner.
  • Support the school-based care team to ensure the student identified as having complex medical care needs has a Care Plan developed in collaboration with the student/family and integrated care team that is reviewed regularly.
  • Complete documentation as required, ensure student records contain all relevant medical information and collect data as needed.
  • Provide comprehensive assessment, intervention and evaluation of outcomes and work with the inter-professional team in the provision of safe, evidence-based quality care.
  • Daily planning, organizing, delegating, coordinating and evaluating optimal nursing care along with troubleshooting day-to-day obstacles within the service.
  • Provide clinical care to students through the effective application of clinical judgement, computer-based decision support software, appropriate evidence-based clinical guidelines, and other relevant information databases/resources.
  • Using in-depth clinical knowledge, apply process to identify student needs, risk and safety issues, educational opportunities, alternate services required and ensure appropriate care pathway.
  • Lead and participate in student care conferences to help coordinate care and services and share information to ensure continuity of care.
  • Support EECD, and family to ensure the appropriate education is provided and or available to the student's school-based circle of care.
  • Build credible and therapeutic relationships with the student/parents, school personnel, Regional Health Authorities, physicians and nurse practitioners, Social Development, other partner organizations and community agencies involved in the circle of care as an ambassador of the program.
  • Communicate effectively and demonstrate cultural competence.
  • Assure leadership, direction and supervision of LPN's clinical work.

REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:

  • Bachelor's degree in Nursing;
  • Actively registered or ability to register with the Nurses Association of New Brunswick;
  • Three (3) years' experience of specialized registered nursing experience in acute care setting with consideration given to those with nursing lead experience in specialty clinics;
  • Canadian Nursing Association certification in a specialty area related to community health is an asset;
  • A good driving record with a valid New Brunswick Class 5 Driver's License;
  • Have a current CPR Level C certification or be prepared to obtain within 3 months;
  • Ability to work independently and effectively with an inter-disciplinary team and possess leadership and coaching skills, strong interpersonal skills, good judgment and decision making skills;
  • Well organized with the ability to prioritize workload in a fast-paced environment and comfortable working to a schedule with demanding turn-around times;
  • Possess sound analytical skills and clinical judgement with the ability to work in a remote work environment;
  • Demonstrated proficiency in Microsoft Office;
  • Support student placement initiatives;
  • Able to work all shifts as assigned, maintaining a good attendance record;
  • A high regard for confidentiality; and
  • Submit a valid and clean criminal record check.
