JOB SUMMARY
NB Health Link leverages technological solutions and innovative clinical delivery models to provide access to primary care services either virtually or in person based on the unique needs of the patients.
Under the supervision of the manager, the resource nurse provides clinical guidance and support to NB Health Link clinical and non- staff in the planning and delivery of care to NBHL patients and their families/support persons.
ROLES AND RESPONSIBILITIES
Clinical Leadership and Patient Care
- Support and evaluates clinical practice, the delivery of care model, etc. while proposing an action plan for implementation of improvements as needed;
- Collaborates in the implementation of best practices.
- Maintains open lines of communication between members of the interdisciplinary team.
- Provides leadership and supports clinical staff with complex clinical situations in accordance with unit policies and procedures.
- Consults in developing and revising the policies, procedures, protocols and any other clinical documentation related to NBHL.
- Promotes interdisciplinary patient centered care approach.
- Communicates with Clinical manager - chain of command needed.
- Attends discipline specific meetings as appropriate
Staff Orientation and Development
- Prepares, coordinates and delivers orientation and continuing education programs focused on skills specific to best primary care practices.
- Ensures the orientation material and training content are up to date.
- Promotes principals of best primary care practice with NBHL staff and supports nurses in developing skills needed to be competent.
- Acts as a mentor with new NBHL staff.
- Assists the clinical manager with identification of educational needs, training needs and opportunities.
- Trains the staff on using new pieces of equipment and techniques relevant to specific practice.
- Collaborates with the development of patient's educational tools and resources required for patients and families' teaching.
- Supports clinical staff in the acquisition of community problem solving / critical thinking skills in an effort to support informed decision-making.
- Participates in caseload review with the manager in an effort to support efficiency and consistency in caseload management as well as meeting best primary care guidelines.
- Collaborates in the implementation of appropriate indicators and ensure that NBHL staff is aware of the indicators.
Quality Improvement and Risk Management
- Collaborates in quality improvement initiatives as required.
- Develop quality assurance processes to ensure policy adherence including chart audits and phone monitoring.
- Recognize trends in quality, risks and incidents reports and develops a plan to address issues;
Research
- Assists in finding potential patients for researchers through the promotion of approved projects.
- Participates in research projects as required.
REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:
- Bachelor Degree in nursing;
- Active registration with the Nurses Association of New Brunswick; and
- Minimum of five years of experience with acute care settings within the last 7 years. Other nursing clinical experience may be considered.
- Demonstrated knowledge of current health trends, principles of primary care, community care, senior, acute care;
- Demonstrated excellent communication, interpersonal and leadership skills which promote a collaborative team approach including ability to be innovative, think critically, group facilitation and problem solving.;
- Demonstrated ability to use computer tools such as Microsoft Office suite (Word, Excel, PowerPoint and Outlook) and various Meditech software applications and understanding of workload measurement system;
- Knowledge of community resources; and
- BCLS Certification.
