JOB PURPOSE

The Immunization Coordinator is a Registered Nurse with knowledge and experience in the field of Public Health. This includes an in depth understanding of key Public Health concepts and principles including health promotion, prevention and protection and application of the Population Health approach. As a member of a multidisciplinary team, the coordinator may be involved with one or more Public Health programs; Healthy Families/Healthy Babies, Immunization, Communicable Disease, Sexual Health and Healthy Learners –to lead, facilitate and organize standardization. The coordinator also plays a key role in driving quality outcomes and to create the harmonization of practices through a shared vision and strategic prioritization.

QUALIFICATIONS

  • Bachelor's degree of Nursing. A relevant master's degree is considered an asset.
  • A minimum of 3-5 years recent Public health experience with at least 1-year recent experience working in the Immunization program.
  • Demonstrates (through learning/experience) knowledge and understanding of Population Health principles and their application to Public Health practice.
  • Demonstrates proficiency in MS Office Suite applications (Word, PPT, Outlook, Excel, and Publisher).
  • Demonstrates leadership skills and an ability to think, analyze and synthesize critically, strategically and creatively.
  • Demonstrates exceptional organizational and prioritization skills and efficient time and work management.
  • Demonstrates exceptional verbal and written communication skills: clarity, transparency and professional.
  • Demonstrates ability to develop and maintain good interpersonal relations through diplomacy and respect for others, persuasion and negotiation skills, appreciation of diversity, and ability to engage and build consensus.
  • Demonstrates personal qualities: motivation, positive attitude, flexibility.
  • Ability to travel within Horizon Health Network as required.
  • Possess a valid driver's license.
  • Possess a valid passport.

  • Good work history (performance and attendance). Current registration with NANB.

KEY RESPONSIBILIIES

The Registered Nurse in this lead role is responsible to:

  • Lead, coordinate, assign and guide the work of the Immunization team(s); which includes ongoing communication, reviewing and ensuring team follows standard guidelines, policies and procedures as it relates to program mandates.
  • Provide support to and collaboration with team members
  • Develop and maintain good working relationships with internal and external partners as well as relevant stakeholders.
  • Facilitate the process of applying evidence informed decision making (research, data, government strategies and mandates) to determine program priorities, influence policy and make recommendations.
  • In collaboration with the team, develop regional goals, objectives, work plan, project charters, guidelines and processes in accordance with the strategic direction of Horizon Public Health, the Regional Medical Officer of Health and the Office of the Chief Medical Officer of Health based on the needs of the community.
  • Work collaboratively with other Public Health programs and Public Health leadership to continuously monitor, audit and track progress and outcomes of the team's work plan through development of dash board and indicators; and provide regular updates to team Manager / Director.
  • Participate in Public Health program committees. Provide local perspective and facilitate regional standardization.
  • Provide leadership in outbreak response. Work in collaboration with the Regional Medical Officer of Health and the Public Health leadership team to develop a regional response.
  • Participates in Public Health Emergency / Critical event response as needed.
