The Senior-Friendly Manager - Acute is a transformational leadership position focused on advancing care for older adults within their zone. Reporting to the Director of X in collaboration with the Strategic Health Network- Health of Older Adults, this role is responsible for leading the integration of senior-friendly care practices and implementing evidence-based initiatives such as Acute Care of the Elderly, Senior-Friendly Emergency Departments, and Senior-Friendly Hospitals to support the establishment of Centres of Excellence in Aging in line with recommendations from Health Accord NL and the vision of the Strategic Health Network for Health of Older Adults.

A key aspect of this role is the establishment and leadership of zone-based working groups to operationalize senior-friendly care priorities. By bringing together key stakeholders and partners, the manager will ensure the effective implementation of initiatives that improve outcomes for older adults. This will require networking and partnering with various programs across the continuum of care including but not limited to: Medicine, Surgery, Emergency Services, Rehabilitation, Primary Health Care, Community Supports, Professional Practice, Clinical Efficiency, Quality, Research and Innovation, Digital Health, as well as various health system partners to advance senior-friendly initiatives.

Additionally, the Senior-Friendly Manager - Acute will lead and/or participate in provincial working groups and contribute insights from their zone to inform provincial strategies and align local efforts with broader system-wide improvements. They will also need to work in close partnership with the Senior-Friendly Manager- Community to support seamless care delivery across the continuum of care for older adults.

The Senior-Friendly Manager - Acute oversees the planning, implementation, and evaluation of senior-friendly care initiatives within their zone. This includes collaborating with multidisciplinary teams to develop standardized care pathways, enhance patient flow, and ensure smooth transitions between acute care and community-based services, long-term care, and personal care homes. As a change management expert, the manager will design and implement strategies to engage stakeholders, mitigate risks, and foster the seamless integration of evidence-based practices. This role also involves developing education materials and providing coaching/ mentoring support to staff to build capacity for delivering high-quality, senior-friendly care.

Resource planning and budgeting are integral to the position. The manager will identify resource requirements, prepare budget submissions, and support recruitment and retention for senior-friendly initiatives/ programs. The manager will also supervise/ manage staff to ensure effective operation and outcomes. They will establish performance indicators and lead the collection, analysis, and reporting of data to evaluate the effectiveness of these initiatives. Insights from these evaluations will be shared with the SHN- Health of Older Adults and provincial leadership to guide decision-making and ensure alignment with provincial priorities. Responsibilities of this position may also include developing and delivering presentations for relevant audiences which may include executive, leadership, providers, and/or patients and families.

By driving innovation, fostering collaboration, and advancing evidence-based practices, the Senior-Friendly Manager - Acute will play a pivotal role in transforming care for older adults and ensuring their zone contributes meaningfully to the provincial vision for Centres of Excellence in Aging and enhanced care delivery for older adults in Newfoundland and Labrador.

Job Qualifications
Education
A baccalaureate degree in a health-related discipline or social sciences is required.
A master's degree is preferred.
Education and training in project management, change management, and quality improvement is an asset.

Experience
A minimum of three years' experience with older adults in acute care required.
Experience in project management and change management.
Experience with quality improvement and/or process improvement.
Previous leadership experience is an asset.
Recent management experience is an asset.

Competencies (Knowledge, Skills & Abilities)
You demonstrate knowledge of the breadth and complexity of the care of older adults during hospitalization. To have good understanding of quality improvement principles, resource/capacity management, and pathway planning.
