Shoreham Village is looking for a Full Time Permanent Social Worker - LTC

Reporting to the Senior Director, LTC.

Qualifications:

  • Bachelors' degree in Social Work
  • Masters' degree in Social Work considered an asset
  • Registered with the Nova Scotia Association of Social Workers
  • Experience in palliative care, dementia care, geriatrics, mental health and/or long term care preferred
  • Must have own vechicle or access to reliable transportation

Major Responsibilities:

  • Coordinates admissions, discharges and transfers.
  • Provides support for residents and family
  • Responds to needs for resources for residents and interdisciplinary team.
  • Participates in corporate initiatives
  • Participates/chairs care conferencing
  • Leads and facilitates quarterly family council meetings
  • American Sign Language considered an asset

Tasks:

  • Serves as a resource for residents and their families advocating with internal and community contacts.
  • Maintins appropriate documentation on medical records.
  • Maintains case notes on residents as needed.
  • Counsels clients and patients in individual and group sessions.
  • Supports and understands needs of people in transition.
  • Collaborates with other professionals to evaluate patients' mental health, medical or physical condition and to assess clients' needs.
  • Declares vacancies and coordinates transfers.
  • Facilitates admissions process (e.g., completes and sends paperwork, communicates with residents and/or family, orientates residents)
  • Provides one to one support to resident and/or families (e.g., discharge information, providing transitional counselling)
  • Provides conflict resolution.
  • Completes lease signing with resident or power of attorney when necessary.
  • Facilitates the completion of documents in accordance with the Personal Directives Act.
  • Completes other tasks as required.

Hours of work: Full Time Day - 80 Hours Biweekly

Start Date: January 19th 2025

Salary: As per Shoreham Scale
