Demands, duties, qualifications
The philosophy of a Family Care Team (FCT) is to deliver care that is patient-centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are, using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client's need. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT's will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers. The FCT for the Baie Verte/Springdale will include the geographical areas of Green Bay and White Bay. Travel throughout the region will be required.

Definition of Work:

As part of the collaborative team, Licensed Practical Nurse I's will be expected to practice to their full scope of practice in order to meet client needs and participate in direct care of clients. The FCT aims to provide continuity of care to improve health outcomes and quality of life through client attachment and client paneling to clinic providers, with the support and wrap around services of the full team. This position will function as a member of the FCT working with physicians, nurse practitioners, registered nurses, and other members of the multidisciplinary team. Responsibilities include but are not limited to health assessment, supporting chronic disease management including the BETTER program, wound care and health maintenance / promotion in consultation and collaboration with the Registered Nurse/ Nurse Practitioner/ Physician. The Licensed Practical Nurse I will report to the Regional Manager for Family Care Team of the designated area.

The Licensed Practical Nurse I will be responsible for documentation using the Electronic Medical Record (EMR). This position supports implementation and evaluation of comprehensive education programs for individuals with chronic diseases as well as their support systems, with the supervision of the Regional Manager and in collaboration with other team members. The role will encompass participating in clinic evaluations and continuous quality improvement processes based upon current best practice guidelines. Flexible hours are required (combination of days, evenings, and weekends required). Travel throughout the region may be required.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

• Constantly moves/lifts up to 10-15lbs, regularly moves/lifts up to 50lbs; required to restrain/reposition/move/lift up to, or above, 50lbs with co-worker assistance and repetitively required to complete pushing/pulling with medium force (22-44lbsF).
• Constantly stands/walks. May work in awkward or cramped positions to assist with care. Work repetitively involves reaching/stretching, bending/twisting and repeated bouts of low-level postures such as squatting, crouching or stooping.
• Potential for exposure to blood, bodily fluids and/or other contagion. May be exposed to clients with potential risk for aggressive behavior.
• Work Environments present potential for exposure to scented/chemical products and/or other allergens within publicly accessible areas.

Required qualifications:

• Graduation from a practical nursing program approved by Central Health.
• Current BLS certification.
• Current registration with the College of Licensed Practical Nurses of Newfoundland and Labrador. Must be able to work at full scope of practice and demonstrate these skill sets.
• Demonstrated proficiency in Wound Care Management and Medication Administration including IM, IV, and Subcut Injections.
• Willingness to complete advanced education in:
o Administration of immunizations.
o Advanced wound care including packing and negative pressure therapy.
o Other additional skill modules as available.
• Demonstrated effective interpersonal and communication skills.
• Demonstrated ability to work independently while effectively working with support of the Community Supports team.
• Willing to work a flexible work schedule.
• Consistent demonstration of Central Health's corporate values; Compassion, Accountability, Respect, Equity, and Safety
• A satisfactory record of work performance and attendance is required for this position.
• A satisfactory Certificate of Conduct from the RCMP or local police authority.
• A valid Newfoundland and Labrador Class 5 Driver's License.

Preferred Qualifications:

• Demonstrated skill in using health care-based computer systems (CRMS, Meditech, EMR).
• Experience in the care and management of patients with diabetes, COPD, heart failure, and/or mental health disorders is an asset.
• Training as a BETTER practitioner.
•Electronic Medical Records (EMR) training is required and will be provided upon hire.
