Definition of Work:
Reporting to the Manager of Primary Health Care and Chronic Disease, the Primary Health Care Facilitator, or Community Health Educator, will utilize the principles of primary health care, population health and community development to facilitate the management of processes related to health promotion, disease prevention, supportive, curative, and rehabilitative services and work in partnership with individuals/groups/organizations and committees to respond to the population served. The successful candidate will work in collaboration with the Managers of Site Operations and leadership team to support the primary health care team/networks and Community Advisory Committees, as well as individuals and groups. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required Qualifications:
• Undergraduate degree in health or one of the social sciences.
• Two (2) year's current experience within the last five (5) years in the areas of community health/community mobilization.
• Demonstrated leadership experience and expertise in team building as well as group facilitation skills.
• Demonstrated knowledge of the psycho-social, biological, economic, and environmental determinants of health and their implications for community health service delivery.
• Demonstrated understanding of tenants of primary health care and provincial PHC framework, as well as a commitment to the community development concepts.
• Demonstrated experience in coordinating/working within an interprofessional team is essential.
• Demonstrated effective communication and analytical skills.
• Use of a private vehicle and valid driver license are a condition of employment.

• Satisfactory record of work performance and attendance is required for this position.
• A satisfactory certificate of conduct from the RCMP or local police authority.
