Demands, duties, qualifications
The Role: Reporting to the Community Support Manager (Western Zone) the incumbent will have provincial leadership responsibility for Dementia Care navigation across the continuum of care and throughout all health service zones. Primary responsibility is providing system wide navigation of programs and services available to people living with dementia, their families, and essential care partners. The Social Worker works to their full scope of practice as a collaborative practitioner within all zonal teams. This includes working across organizations, programs and zones to create pathways for Dementia Care navigation and providing client-centered support to clients and essential care partners. This position requires considerable health system and community collaboration to achieve a holistic and client-centered approach.
Qualifications: Master of Social Work preferred, supplemented by relevant work experience in the field of Dementia Care. Currently registered or eligible to be registered with the NL association of Social Workers. Minimum three-five (3-5) years clinical experience with knowledge in the field of Dementia Care.
Experience in navigating both the health sector environment and various social systems with which clients intersect is an asset. Extensive knowledge of both internal and external community resources and working with a seniors' lens is an asset. Excellent interpersonal skills, numeracy skills, communication skills (oral and written), organizational and analytical skills. The ability to establish and maintain effective working relationships to foster teamwork and lead others to achieve results. Work with a high degree of independence and professionalism and be able to work with diverse groups, including people living with Dementia, NLHS staff and physicians, community groups, government departments and project teams. Ongoing professional development participation is required. Basic proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other electronic health information systems such as Meditech, CRMS, Healthy NL, etc. Proven leader with sound decision-making skills and professional judgement. Supervisory, teaching and/or community experience is an asset. Good standing in attendance and work performance requirements due to leadership role and inability to provide relief.
