Horizon Health Network (Horizon) is seeking a Regional Manager Population Health who is a strategic, logical, detailed focused, diverse, and adaptable individual to join our team. The ideal candidate will have experience in the health care system including experience in a leadership role with a lens of health equity, using a systems thinking approach and working towards addressing the social determinants of health within a complex health structure.

Who you are
- You have a worked in the health care system, including experience in a leadership role for a minimum of 5 years. You possess a minimum of an undergraduate degree in a health or social science discipline. You have additional training in project management and organizational change/improvement (an asset). You also possess additional experience leading collaborative initiatives in a clinical setting (asset). A Masters degree is also an asset, and you have the ability to work with minimal supervision and possess strong organizational and time management skills.
You have experience in:
- a facilitation model, change management, and quality improvement which are assets;
- translating data in knowledge sharing activities; along with experience in complex report writing and presentation skills;
- executing organizational culture change;
- advanced communication skills including demonstrated (excellent) presentation skills with diverse audiences essential (e.g. executive leadership team, executive boards-provincial and national in scale, community led groups, non-profit sector, etc.);
- working within a complex and rapidly changing environment;
- negotiating, informing and shaping strategic direction;
- moving knowledge to action;
You have excellent proficiency in Microsoft Office applications.
Written and spoken competence in English is required at an advanced level.
What you will do
- Provide strategic leadership to Horizon Health Network in the adoption of evidence-informed approaches to improve population health and effect system transformation;
- Identify and support research, best practices and pilot projects that align with the mandate and strategic direction of the department;
- Lead and support interprofessional and inter-departmental project teams in the achievement of identified priorities and objectives;
- Develop and integrate key performance indicators related to health equity and population health initiatives;
- Advocate for support of priorities as identified by the communities served by the organization;
- Identify, analyze and disseminate population health data and information to inform organizational direction, decisions and resource allocation;
- Collaborate with others in Horizon, the New Brunswick Department of Health and New Brunswick Health Council to establish innovative ways to assess and address health inequities in New Brunswick;
- Travel throughout the region as required;
- Performs other duties as
The Regional Manager Population Health will oversee the development and implementation of strategic region-wide initiatives that address barriers to achieving equitable health outcomes for the populations Horizon Health serves. These initiatives, borne from the identified needs of our communities, will support the department's mandate "We transform systems, build capacity, and empower communities to improve the health of the population and promote health equity"
More specifically, the Regional Manager Population Health will apply a systems change approach to align thinking, enhance relationships, and improve measurements to successfully address the regional priorities identified by the communities we serve.
Within this term, addressing strategic priorities related to health equity and the social determinants of health by building capacity in the organization will be a primary focus of this position. The successful candidate will largely focus on facilitating the development of a Health Equity Framework for Horizon, and support other key foundational initiatives as assigned by the Regional Director. This work will be accomplished by building successful partnerships with key leaders within our organization, community partners, other government departments and non-profit agencies. Other term initiatives may include support of overall program data initiative and KPI work; report writing on requested items and other duties as assigned by the Regional Director.
