JOB SUMMARY

To holistically coordinate the care of each patient, incorporating the nursing profession's values of knowledge, theory, and practice. To assess, plan, implement and evaluate the needs of patients and significant others in the delivery of care and collaborate with all members of the interdisciplinary team in this approach.

The incumbent will be part of the Special Care home (SCH) unit and will provide support to the respective SCH in their regions. The goal of this team will be to ensure enhanced clinical support to the residents in the special care home by acting as a liaison nurse, providing vaccinations on site, coordinating care and performing the following tasks:

  • Assessment of each resident in the SCH. During the assessment, staff will complete EMP forms, review the resident's chart, complete required requisition forms, review patient medications and continually re-assess the needs of the client
  • Patient Care: provide oversight to ensure updates to the patient Care Plan, as required; teach/educate SCH staff or EMP LPN on individual care; identify gaps and coordinate referrals (to EMP other professionals or foot care, etc; identify extra support that might be required
  • Collaboration with the SCH, Social Development and any other organizations as required
  • Education: provide education to the resident and their circle of care as required and education to the SCH staff (ex: on Restore2mini, how and when to use it)
  • Communication: communication will be a key component as EMP staff will need to communicate with several partners such as the Primary Care provider or Nurse practitioner, the client and their circle of care; the staff at SCH; and external agencies as needed (i.e. contact SD to re-assess a resident if their needs change; refer client to the Mental Health clinic, etc).
  • Coordinate vaccination campaigns in the SCH and provide the vaccination to the residents of the SCH

In the event of an outbreak (Covid or other), the incumbent will provide the clinical support and quality centered care to the residents on site (assess the resident as needed) but will not be staying on site. Travel within the incumbent's region might be required at this time. Travel within the Province might be required for vaccination in the SCH.

REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:

  • Diploma in Nursing or a Bachelor Degree in Nursing;
  • Must be registered (active) or ability to register with the Nurses Association of New Brunswick;
  • A minimum of two years in the last five years of specialized nursing experience in the areas of family practice, surgical, palliative care or oncology in a patient unit that requires daily collaboration with various health disciplines and family members as part of reaching patient goals;
  • Knowledge of the target population of Extra-Mural Services and available community services;
  • Must have a valid New Brunswick Class 5 Driver's License and good driving record (proof required);
  • Submit a valid and clean criminal record check;
  • Have a current CPR Level C certification that has been issued in the last 12 months;
  • Must demonstrate the ability to work efficiently as a member of the team and possess leadership and coaching skills, strong interpersonal skills, good judgment and decision making skills;
  • Must be able to work all shifts as assigned, maintaining a good attendance record;
  • Must have a high regard for confidentiality; and
  • Must have excellent work habits and have a professional appearance.
