JOB PURPOSE

The successful candidate will be responsible for promoting, facilitating, and exemplifying the provision of high-quality care as a Primary Health Care NP within their legislated scope of practice as outlined in the provincial Nurses Act, NANB Entry-Level Competencies for Nurse Practitioners, and the NANB Standards for the Practice of Primary Health Care Nurse Practitioners.

As a member of the interdisciplinary team, the NP provides direct care to scheduled and unscheduled patients across the lifespan with a variety of health conditions. The NP evaluates patient outcomes, provides consultation to, and consults with other team members. The NP also utilizes research, leadership and educator knowledge and skills to support/provide health promotion, illness and injury prevention, and community development initiatives.

QUALIFICATIONS

  • Active registration as a nurse practitioner with the Nurses Association of New Brunswick.
  • Current BCLS.
  • Ability to work independently and in teams; to lead and drive change.
  • Demonstrated organizational and leadership skills.
  • Demonstrated understanding of privacy and confidentiality, evidence-based nursing practice and mentoring cultures.
  • Excellent interpersonal, oral, and written communication skills.
  • Good work and attendance record.

KEY RESPONSIBILITIES

  • Maintains an awareness of current trends, issues, and concepts within provincial and national health care, and the NP profession.
  • Contributes to process change involving NP practice, for the purpose of increasing efficiency and effectiveness of health services.
  • Participates in the development, monitoring, and evaluation of evidence-based protocols/guidelines/standards of care for geriatric patient populations, in collaboration with team members.
  • Acts as a consultant to registered nurses, licensed practical nurses, and personal care attendants (resident attendants) as it relates to diagnosing, ordering of tests, and prescribing medications as outlined by the NANB.

Health Assessment and Diagnosis

  • Completes a comprehensive health assessment
  • Orders, obtains, performs and interprets diagnostic and laboratory screening tests in accordance with the NANB Standards for PHC NPs and within legislation.
  • Establishes a diagnosis.
  • Initiates appropriate treatment plan in collaboration with the patient/client/family or substitute decision maker and other team members as indicated.

Therapeutic Management of Acute and Chronic Conditions

  • Demonstrates knowledge of current best-practice guidelines for the delivery of care.
  • Prescribes, adjusts and renews medications in accordance with legislation, NANB and employer policy.
  • Monitors, evaluates, and alters therapies and/or treatments as appropriate.
  • Initiates timely referral or collaboration with other team members, as indicated.

Health Promotion and Prevention of Illness and Injury

  • Demonstrates knowledge and understanding of health promotion services and facilitates primary, secondary and tertiary prevention strategies.
  • Incorporates risk reduction and disease and injury prevention strategies.
  • As part of the interdisciplinary team, participates in the development and implementation of community/population health interventions as required.

Professional Responsibility and Accountability

  • Maintains the competencies as outlined in the NANB Standards of Practice for Primary Health Care Nurse Practitioners.
  • Actively participates as a team member to provide comprehensive care to all patients.
  • Exemplifies and promotes excellence in advance practice nursing care.
  • Utilizes nursing and other research to support clinical knowledge and evidence-based practice.
  • Establishes a network of referral sources to accommodate the provision of care to patients. Referral sources are internal and external to the organization.
  • Functions as a resource, preceptor and mentor to Nurse Practitioner students and other health professionals.
  • Assumes responsibility for self-directed professional development and participates in professional education activities.
  • Participates in the development, monitoring, and evaluation of evidence-based protocols/guidelines/standards of care in collaboration with team members.
  • Participates in regular Drug Reviews, Professional Advisory Meetings, Ethics Meetings and Multidisciplinary Care Conferences, or other committees as required.
