Horizon Health Network in beautiful New Brunswick is looking for an Indigenous Patient Navigator to join our team!

Horizon Health Network (Horizon) is seeking a Passionate Dedicated and self-motivated individual to join our team as an Indigenous Patient Navigator. The ideal candidate will act as a resource and point of contact for both Indigenous clients and their families, healthcare providers and when appropriate, community members, to ensure the provision of care is culturally safe and client centered. In collaboration with patients & families, healthcare providers and community resources, the Indigenous patient navigator will navigate the healthcare system by facilitating access to services and assist during discharge planning. Using a Two-Eyed seeing approach, Indigenous patient navigators foster an environment that supports traditional Indigenous ways of knowing & healing practices with evidenced based quality care. The goal of this position is to ensure Indigenous clients of Horizon Health Network are accessing appropriate care from appropriate sources, increase client capacity and connections to be stable and supported.

Who you are

  • One (1) to three (3) years' recent related experience working in Indigenous health or social programming
    • Experience advocating for clients
    • Experience prioritizing work in an evolving fast paced setting
    • Proficiency level in Microsoft Office applications.
    • Possess strong mediation skills and ability to resolve conflict
    • Exceptional interpersonal skills and ability to foster trusting relationships
    • Understanding and knowledge of Indigenous healing practice and person-centered care and able to
    conduct, assist with or facilitate smudging ceremonies
    • In-depth knowledge of Indigenous Health programming at the systems, community, provincial and
    federal level
    • Demonstrated knowledge of local Indigenous peoples' demographics, languages, cultures, customs,
    and traditions.
    • Ability to speak Wolastoqey or Mi'kmaq an asset
    • In-depth Knowledge of cultural safety and Two-Eyed seeing within healthcare and ability to apply
    those concepts in their work.
    • Demonstrated understanding of Canadian colonization, Indigenous systemic racism and its impact on
    the current health and wellness of Indigenous peoples
    • An ability to facilitate and/or make referrals as needed
    • Applicants must be of Indigenous descent
    • Demonstrated ability to function independently, be self-directed and to make competent decisions in a
    highly complex environment
    • Written and spoken competence in English is required.
    • Ability to travel throughout the region as required.
    • Physically able to perform assigned duties.
    • A valid driver's license and the use of a vehicle

Education required
• Undergraduate Degree in a health or social sciences discipline preferred
• Additional Certifications in health such as trauma-informed care, birthing doula or death doula or
willing to obtain

What you will do

  • Support the facilitation of a Two-Eyed Seeing approach to wellbeing and care of Indigenous clients
    Horizon Health Network facilities.
    • Visit and engage with Indigenous clients and families to support a positive hospital experience.
    • Provide advocacy & mediation, assist with health literacy and empower clients to actively participate
    in health decisions.
    • Promote traditional cultural practices in health and healing by facilitating access to cultural,
    spiritual, and linguistic resources.
    • Provide consultation and navigation support to Indigenous clients, their families, and their healthcare
    team by strengthening hospital staff awareness of Indigenous community resources through formal and
    informal education and/or mentoring opportunities.
    • Participate in case conference meetings and discharge planning for Indigenous clients who are
    returning home or transitioning to long term care as required.
  • Accurate documentation in patient records
  • Collaborate with community rightsholders & stakeholders (community health centers, non-Insured
    health Benefits) to ensure seamless transitions during and from acute hospitalization Facilitate and or
    make necessary referrals as needed.
    • Possess and maintain a strong awareness of Indigenous community resources.
    • Develop written resources for patients and the healthcare team.
    • Provide education sessions to staff, physicians and community partners on Indigenous Health
    Programming in Horizon and Indigenous patient health needs.
    • Work alongside community partners and our internal colleagues to support understanding and
    awareness of cultural safety and racism.
    • Assist patients to navigate the complaints process to ensure appropriate follow-up.
  • • Develop and complete daily and monthly statistical workload measurement requirements which are
    submitted regularly to the Indigenous Liaison Committee and Internal Horizon Health departments.
