JOB DESCRIPTION
As a member of the interdisciplinary team this position provides quality diabetes care within the scope of clinical nutrition care to patients, families and caregivers in both in-patient and out-patient settings. Responsibilities include but are not limited to provision of diabetes education, care and services, teaching diabetes education classes, presentations to community and professional groups and mentoring students assigned to the clinic. This position strives to improve health outcomes with best practice by enhancing clinical management, patient knowledge and self-management skills.
QUALIFICATIONS
- A Bachelor of Science degree in nutrition or dietetics and successful completion of an accredited internship program.
- Must be registered and a member in good standing with the New Brunswick Association of Dietitians.
- Certified Diabetes Educator (CDE)
- Insulin Dose Adjustment Certification considered an asset
- Ability to attend work on a regular basis.
- Ability to perform the duties of the position.
- Good work record.
KEY RESPONSIBILITIES
The Dietitian is responsible for:
- Full clinical assessment establishing a baseline for intervention as well as ongoing monitoring and evaluation for clients with any form of diabetes.
- Establishing effective care plans (in collaboration with primary health care provider(s) as indicated), provision of intensive counselling, education and advocacy as well as make recommendations for appropriate treatments, referrals or changes to therapy.
- Effectively demonstrating strong clinical skills in chronic disease management through daily assessment, interventions and through excellent interpersonal communication.
- Liaise with internal and external agencies to support sourcing and funding for diabetes management products.
- Collaborating and maintaining productive relationships with managers, physicians, clinic staff, patients, and families.
- Access or develop resources to support diabetes self-management.
- Supporting the patient / family in the self-management of diabetes.
- Supporting clinicians external to the diabetes clinic on best practices for diabetes management.
- Maintaining clinical records and documentation
- Preparing and presenting information and education sessions
- Providing mentoring and support for a variety of post-secondary students / interns.
- Participating in program development, including developing, maintaining and evaluating policies and procedures.
