Job Title: Indigenous Patient Navigator, Full Time, Permanent Position

Location of Position: Corner Brook, NL
Hours of Work: Monday to Friday, 35 hours per week, 9:00 am to 4:00 pm
Rate of Pay: $48,000.00 per annum
Job Description: The Indigenous Patient Navigator provides advocacy, support, and a point of contact for individuals and families who are receiving care within the provincial health system. The Navigator uses their network to remove barriers for the client to ensure they can access appropriate healthcare services. Patient Navigators provide navigation to existing healthcare services while connecting members to other community health programs and supports.

The Indigenous Patient Navigator's mandate is to assist individual Indigenous people with access to appropriate healthcare while providing advocacy at the community level. As a personal guide, the Navigator is there to help the patient and family through their healthcare journey. The Navigator acts as a bridge between patients, their community, and the health care system through consultation and referral services.

Responsibilities:
The Indigenous Patient Navigator will work collaboratively with departmental staff and external healthcare networks to ensure that individuals, families, and community members are able to access and navigate appropriate health services. Additional responsibilities are as follows:
• Assist First Nation, Inuit, and Innu clients in navigating and addressing issues in the health care system. This includes providing patient advocacy and assisting with the navigation of healthcare related programs and services.
• Assist in removing barriers and challenges for clients to access appropriate healthcare services.
• Maintain a good working relationship with external stakeholders, such as government agencies, service providers, and other organizations.
• Act as public educator for the Band by carrying out presentations pertaining to healthcare navigation and advocacy.
• Assist in advocating for culturally relevant and appropriate services and supports within the healthcare system as a whole.
• Consult with health professionals and service providers regarding individuals' access to services.
• Liaise and advocate for client's health and wellness supports.
• Liaise with the individual's external supports, as required.
• Respond to general patient questions related to healthcare systems and the navigation of these systems.
• Attend appointments with patients if requested by the patient.
• Assist individuals with completing healthcare related paperwork such as the Non-Insured Health Benefits (NIHB) reimbursement documents.
• Assist in connecting the patient and their family with community cultural supports.
• Maintain confidentiality and protect the privacy of members, internal operations, and activities of the Band.
• Provide reports and keep records as required by Qalipu and Indigenous Services Canada.
• Ensure files are stored and maintained as per the Canadian Privacy Act, Personal Information Protection and Electronic Documents Act (PIPEDA), Qalipu Electronic Records Management System, and Quality Management Protocols for the Health division team.
• Other duties as required.

Qualifications:
Education: Post-secondary education in a relevant health or social sciences field. This may include a four-year university degree or a 2 or 3-year college diploma.

Experience: The successful applicant will have 1-2 years of recent experience working with diverse clients with health issues and social barriers. This includes experience in direct service community-based delivery systems with similar duties and responsibilities.
Working Conditions: This position will have duties predominantly met during regular business hours. Some Travel and overnight stays may be required at times. Additional travel and flexibility of work schedule may be required. A driver's license and access to a personal vehicle is a requirement of the position.
Full-time Employee Benefits:
Annual Leave: FT employees - Three weeks per year.
Statutory and Non-Statutory Holidays: Employees are entitled to 13 days off to cover Statutory and Non-statutory holidays per year.
Summer Office Hours: Office hours during the summer months are from 8:30 am to 4:00 pm Monday to Thursday, Friday 8:30 am to 12:30 pm. The schedule starts the first long weekend in May and continues until the last week of September.
Christmas Break: the office closes down during the Christmas break; this is an added bonus that employees are entitled to.
FT Employee Benefits: Option to enrol in C of C - Employee Benefits Plan.
Registered Pension Plan: QFN has a pension plan, FT employees contribute from 4 to 7%, employee contributions are matched by the Band.
FT Sick and Family Medical: Full Time Employees are entitled to 12 paid sick and family medical days per year.
Under 2 hours Medical Appointment: Employees may attend an appointment of less than 2 hours with no deductions to Sick and Family Medical.
Discretionary Bereavement: One day off per year for the death of a relative not included as immediate family, a close friend or neighbour.
Volunteer Program: Employees may volunteer outside of the organization up to 21 hours.
