About the Role:

Reporting to the Manager of Public Health, the primary purpose of this position is to perform professional community health nursing in carrying out community health programs in homes, schools, clinics and other clinical and community settings. This position provides nursing care; assists in the assessment of community health needs; case management; conducts school health programs; provincial immunization programs and health promotion activities; liaises with other community and professional personnel. The position also provides Home and Community Care Nursing care and participates in the palliative care program, short term acute program and ambulatory care clinics. The position also provides diabetes education and collaborates with other health care professionals in the coordination, planning, implementing, and evaluation of diabetes clinical services. The incumbent is committed to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks, and implementing improvements.

Qualifications:

? Graduation from an approved/recognized school of nursing program is required;
? Current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required;
? Graduation from an approved Bachelor of Nursing degree program preferred;
? CNA certification is an asset;
? Current CPR;
? Minimum of three years clinical experience in nursing; one year of Community Health Nursing (Home and Community Care and/or Public Health) experience is preferred;
? Certification as a diabetes educator, completion of a Diabetes education program with willingness to complete exams when eligible, or a willingness to start a Diabetes education program within 6 months and work towards certification;
? Preference will be given to applicants with the Canadian Diabetes Education certificate;
? Maintain Canadian Diabetes Educator Certification during employment;
? Breastfeeding education/course;
? Wound care management course completed or willingness to complete in a timeframe identified by the employer;
? A valid Newfoundland and Labrador Class 5 Driver license and access to a private vehicle is a condition of employment;
? Applicants must also be prepared to demonstrate an ability to be self-directed and exercise independent nursing judgment in a community based environment with limited supervision;
? Experience working within an interdisciplinary team; with program development and implementation, client teaching and knowledge of adult learning principles is an asset;
? Ability to assess community needs and assets; to develop, implement and evaluate community health nursing programs and healthy public policy in a population health framework;
? Ability to communicate effectively orally and in writing;
? Be able to apply basic computer skills
? The successful applicant will be required to submit a satisfactory Certificate of Conduct.

Duties: (Includes, but not limited to)

? Works with individuals or community groups in planning, developing, implementing and evaluating client focused wellness and prevention programs and diabetes care;
? Provides childbirth education through individual counseling, group teaching and involvement with Health Baby Club and other prenatal groups;
? Breastfeeding support;
? Carries out Public Health programs (such as Healthy Beginnings, Child Health Clinics, immunizations) in accordance with the program plan and following Public Health guidelines;
? Provides communicable disease follow-up according to the Provincial Guidelines;
? Utilizes the Model for the Coordination of Service for Children and Youth;
? Delivers school health programs and wellness promotion;
? Carries out the Home Care and Continuing Care programs, which may include being available after hours and weekends;
? Delivers professional nursing services to clients who are discharged from hospital or referred from the community who require nursing care;
? Works in collaboration with an interdisciplinary team to manage, follow up and evaluate clients' plan of care;
? Provides diabetic teaching and follow up according to current clinical practice guidelines for diabetes;
? Provides support, education, teaching and advice to clients and families who are using the insulin pump;
? Acts as a resource to staff and provides support and information on diabetic education, management and prevention;
? Practices to full scope as outlined by Labrador Grenfell Health's Policies and Clinical Practice Guidelines;
? Maintains continuing education and keeps up to date with evidence-based practice;
? Participates in research, committees, program related activities and policy development as required;
? Assists in the orientation of new personnel and acts as a preceptor for students;
? Maintains accurate electronic records and submits reports and statistics as required;
? Travel may be required throughout the region;
? Flexible hours of work to accommodate Community Health, Home and Community Care and diabetes programs;
? May be required to work alone;
? Maybe required to do relief;
? Other related duties as required.
