Northwood is currently accepting applications for a Clinical Dietitian
Reporting to the Manager, Food and Nutrition Services, the Clinical Dietitian develops individualized nutrition care plans for residents.
Responsibilities:
- Initiates a resident focused nutrition care plan for all residents, comprised of assessment, implementation, evaluation and ongoing monitoring which incorporates evidence based research
- Provides interpretation of the nutrition care plan to resident, other members of the health care team and family
- Participates as a core member of the resident care planning conference team
- Conducts swallowing assessments and meal time observations
- Develops and maintains communication links with multiple internal and external clients
- Consults with nursing staff and members of the interdisciplinary team to establish interventions for the individual nutrition care plans developed / prescribed by the Clinical Dietitian and offers an opportunity for the resident and substitute decision maker to contribute and confirm the nutrition interventions
- Attends family/case conferences and interdisciplinary meetings
- Ensures nutrition care programs comply with established legislated standards and guidelines for LTC by completing audits
- Provides leadership to Resident Support Supervisor, Food Service Supervisors and Dietary Aides specific to implementation of individual resident nutrition care plans
- Maintains/ updates clinical component of database
- Facilitates clinical care which impacts on infection rates, wound care, bowel care, medication use and quality of life
- Mentors dietetic interns and other students
- Provides staff education within departmental resources
- Performs other related duties as require
Qualifications:
- Minimum two years of experience in geriatric nutrition
- Bachelor of Science in Human Nutrition or equivalent from an accredited university
- Completion of a Dietitians of Canada recognized internship program
- Licensure by the Nova Scotia Dietetic Association
- Previous experience in dysphagia management and assessment considered an asset
- Previous experience in mentoring and educating considered an asset
- Computer proficiency required including the use of Microsoft Office
- Knowledge of nutrition interventions used in long term care, including: Dementia, dysphagia, diabetes, COPD and HTN, Treatments/assessments (e.g. bedside swallowing assessments and texture modification), wound care management, and enteral nutrition
Hours: 80 Hours Biweekly
