Horizon Health Network (Horizon) is seeking a strategic, passionate, and driven individual to join our team as a Process Improvement Facilitator. The ideal candidate will have experience driving a culture of continuous improvement through leading and facilitating projects.

Who you are

You have a worked in healthcare or other industry leading projects for a minimum of 6 years with progressive work experience. You have experience in project management, group facilitation, and change management. You have a proficiency level in Microsoft Office applications. You have strong analytical, organizational, problem solving, and communication skills.

Education requirements:

  • University degree in Business Administration, Science, Engineering or a related discipline with six years of progressive work experience
  • Completion of Lean Six Sigma Green Belt or Black Belt training

Written and spoken competence in English is required at an advanced level.

What you will do

  • Act as a facilitator to guide project teams through a structured methodology for process improvement
  • Work with project teams and stakeholders to identify key performance indicators for the ongoing measurability and monitoring of implemented performance changes
  • Ensure appropriate monitoring and reporting of project progress and results
  • Conduct regular post-project reviews of performance change sustainability; offer support to help the department/program get back on track, if required
  • Mentor and coach Horizon employees who are leading Green Belt and White Belt projects
  • Deliver White Belt training to Horizon employees
