Demands, duties, qualifications
DUTIES: Reporting to the Community Support Manager (Western Zone) with the lead for Home First Philosophy of care, the incumbent is a member of the Acute and CS interdisciplinary teams and participates in the planning, development and provision of Social Work services for seniors across the continuum of care. Focus is to support the implementation of Home First philosophy regionally across branches including initiatives to decrease utilization of ALC, improve uptake of Home First initiatives in Emergency Departments, Long-term care and communication strategies related to Home First including physician engagement. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors' health services within health care and the community and providing collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of all individuals (includes patients, families, groups, populations, and communities). This position includes case finding activities and considerable community collaboration. Main overall categories of duties are case management, patient diversion / navigation, and seniors care leadership.
QUALIFICATIONS:
- Master of Social Work required. Other relevant education and experience may be considered.
- Currently registered or eligible to be registered with the NL Association of Social Workers.
- Minimum three-five (3-5) years clinical experience with knowledge from both Patient Services and Community Support program areas.
- The applicant would practice as an independent practitioner as a part of an interdisciplinary team. A high level of independent practice may require additional training and/or study in clinical practice sub-specialties regarding clinical practice.
- Experience working with the geriatric population within a health sector environment is an asset.
- Ongoing professional development participation is required.
- Basic proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other electronic health programs such as Meditech, CRMS, Healthy NL, etc.
- Travel is required. Must hold a current NL Driver's License and vehicle condition of employment.
- Extensive knowledge of both internal and external community resources
- Proven leader with good decision-making skills. Supervisory, teaching and/or community experience is an asset.
- Good standing in attendance and work performance.
- Physical capabilities to perform the duties of the position.
A satisfactory record of work performance and attendance is required for this position.
