Demands, duties, qualifications
Currently reporting to the Manager of Primary Health Care, this position offers an excellent opportunity to apply advanced clinical, theoretical and decision-making skills through comprehensive nursing knowledge and an interdisciplinary collaborative team approach to primary health care. This position includes but is not limited to a preventative, health promotion, supportive, rehabilitative and curative model of care. The incumbent is responsible for maintaining professional and organizational standards.
Qualifications
:
- Current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required;
- Completion of Nurse Practitioner Program is required;
- Masters of Nursing is an asset;
- Comprehensive knowledge and ability to apply the principles of primary health care;
- Ability to work independently and autonomously exercising considerable clinical judgment and initiative in performing duties;
- Experience working within an interdisciplinary team, client teaching and knowledge of adult learning principles is an asset;
- Minimum of five years clinical experience in last seven years of nursing;
- Ability to work independently with little supervision;
- Current BCLS certification is required;
- ACLS, PALS, TNCC or willingness to complete in one year;
- Satisfactory certificate of conduct;
- A valid Newfoundland and Labrador Class 5 Drivers license;
Duties (Include but not limited to):
- Works collaboratively with the Home First Team, and provides PHC services to the Long Term Care and Personal Care Home
- Arranges and conducts primary health care clinics independently and collaboratively assessing, diagnosing, and prescribing in accordance with professional, provincial and organizational standards, policies and clinical practice guidelines;
- Responds to emergency situations and participates in emergency care when required;
- Coordinates treatment plans and follow up based on comprehensive assessments, client and family response and outcomes, collaborating with physician if required care is outside of provider's scope of practice and competencies;
- Prescribes medications in accordance with professional, provincial and organizational standards, policies and clinical practice guidelines;
- Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with Client and/or other team members within the circle of care;
- Monitors client's response to prescribed and discontinued therapies and documents these within client's permanent health record;
- Documenting accurate reports of data collection, interpretation, planning, implementing and evaluating care in accordance to CRNNL documentation standards, LGH's policy and units standards;
- Acts as a liaison/resource with community health providing education and recommending monitoring regimes as required;
- Works as part of the Chronic Disease Management team planning, developing, implementing and evaluating client focused care and follow up;
- Advocates for clients, assessing their needs and if applicable, referring to appropriate care providers, professionals or other services in the interest of the continuum of care;
- Ensures the delivery of professional nursing care to clients utilizing an evidence-based nursing practice approach;
- Recognizes ethical consequences of decisions and applies ethical principles when proposing sound solutions to complex issues;
- Demonstrates progressive clinical leadership skills, conflict management, professional development, interdisciplinary team building, communication and computer skills;
- Ensure quality assurance standards are maintained;
- Recognizes cultural diversity;
- Utilizing, supporting, facilitating or participating in research relevant to nursing and the multidisciplinary team;
- Participating in program and policy development as required;Attends committee meetings related to Nurse Practitioners as requested;
- May be required to work alone;
- May be required to be on call;
- Other related duties as required
Financial incentives may be available and can be combined including:
- Signing bonus up to $16,000
