Through a new, innovative partnership with Nova Scotia Health (NSH), we are opening the first transitional care center in our province, in West Bedford. Shannex is the operating partner for the transitional care centre on behalf of Nova Scotia Health. You will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in central zone, who no longer require hospital care, but need services before they can safely return to their home and community.

We are searching for a Transitional Care Allied Health Manager to join our Stratford Transitional Care Centre team based in Bedford, Nova Scotia.

The transitional care model supports NSH patients/families who require additional time, services, and support to transition back to home/community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation.

The Transitional Care Allied Health Manager provides leadership in an interdisciplinary setting while providing direction in the maintenance of the patient-directed philosophy. The Transitional Care Allied Health Manager coordinates the integrated operations of the facility and oversees the effective and efficient use of allied health resources to provide quality health care.?

This role is integral in supporting the team, patients and families meet the goal of the patient returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise, using effective communication strategies, shared decision-making, and mutual support.

The Transitional Care Allied Health Manager supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.

Key Duties & Responsibilities Include:

  • Participates in the selection, leadership and development of the team
  • Provides guidance and leadership to ensure compliance to policies, procedures, and applicable legislation.?
  • Supervises, directs, and evaluates the work of the team.?
  • Provides ongoing training and education.?
  • Ensure that all work performed is conducted in a safe manner in accordance with Occupational Health and Safety procedures.
  • Takes every reasonable precaution in all circumstances to ensure the health and safety of team members and all others in the workplace, including but not limited to, providing supervision, training, information, and equipment needed for employees to do their job safely.
  • Develops strong relationships with patients, families, substitute decision makers, physicians, other care providers and the care team.
  • Completes required documentation according to established standards
  • Provides direct Nursing care to Patients

Requirements Include:

The selected candidate will have current registration with a provincial licensing body, and a minimum of 5 years experience. You will be service minded, results oriented, and positive towards change and innovation. You will be a team player who works collaboratively to deliver results and also have the ability to work autonomously. In addition, the selected candidate will have:

  • Bachelor of Science in Occupational Therapy, Masters Degree in Physiotherapy; or other related field
  • Must have an active practicing license with provincial regulatory body and be a member in good standing
  • Knowledge of the principals of supervision, organization and administration.
  • Knowledge of operational and personnel rules and regulations, collective agreements and regulations governing long term care.
  • Above average communication and documentation skills.
  • Proficient with computer programs
  • Knowledge of Program Standards, Standards of Practice and applicable legislation (i.e Protecting of Persons in Care Act, Adult Protection, Occupation Health etc.)
  • Demonstrated ability to resolve conflict, manage change, and navigate complex social situations
  • Strong interpersonal skills
  • Demonstrated supervisory skills and ability to evaluate employee job performance.
  • Interpersonal Communication Skills
  • Problem Solving and Independent Judgment

Assets:

    • Previous experience facilitating admission and discharge for vulnerable populations;
    • Focus on facilitation of seamless transitions across health care settings, with a goal of home first;
    • Experience navigating complex social situations, identifying resources and implementing care plans to give the patient/family every opportunity to be successful.
