We are searching for a Permanent Full Time Resident and Family Coordinator to join our Maplestone Enhanced Care team based in Halifax, NS.

About the Opportunity

  • Prepare reports, records, statistics and other data as needed or requested.
  • Participates as member of the interdisciplinary team in team, client and family meetings.
  • Collaborates with clients and family/friends to meet their program expectations and needs.
  • Conduct tours of the facilities and interviews prospective clients and/or families through home, hospital and/or office visits to obtain pre-admission information.
  • Coordinates admission, transfer and discharge of clients in coordination with the interdisciplinary team.
  • Prepares funding requests and reports relating to client needs; maintains client record as required.
  • Promotes safety culture (or act as a safety role model) by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace.
  • Ensures client safety by following and applying all standard operational procedures.

About You

  • Licensed Practical Nurse Diploma or other applicable education
  • CPR certified
  • Previous experience working in long term care or with seniors.
  • Knowledge of the principles of human behaviour and ability to apply program solving skills.
  • Strong computer literacy skills.
  • Ability to communicate effectively and efficiently (both verbally and in writing).
  • Knowledge of available community program and supports.
  • Ability to establish and maintain effective, cooperative working relationships with staff, families, clients and other stakeholders.
