SCHEDULE: Day, 8 and 12-hour shifts. Work schedule may be changed to meet operational needs.

JOB SUMMARY:

Reporting to the Corporate Director of Logistics and Patient Flow and in close collaboration with local staff managers, the Director of Patient Flow is responsible for planning, implementing and managing day-to-day operations in the local zones and in the regional hospital facilities with regard to the patient flow process, coordination of access to beds, discharge planning and collaboration with various outside partners. Committed to patient-focused care, the Director of Patient Flow provides local leadership for a coordinated and interdisciplinary approach to discharge planning, thus creating a culture that fosters the right care at the right place by the right provider. The incumbent will take part in other projects if they are to be aligned with the priority areas identified by the Corporate Director of Hospital Activities. Quality assessment and improvement activities will be essential to achieving the positive results required of this function. The local scope of this role requires the development of strong collaborative relationships with administrative, clinical and medical managers, and the community partners identified.

REQUIREMENTS:

  • Undergraduate degree in health science and/or applied science;
  • Graduate degree in a health-related field would be an asset;
  • At least eight (8) years' experience in health care at a management level;
  • Progressive leadership in management eight (8) years in the areas of leadership, negotiation and mediation;
  • Experience in strategy development, change management, building relationships and partnerships, and achieving results;
  • Experience in data analysis and improvement methodology, and proven ability in making data-driven decisions to improve processes;
  • Excellent skills using the Microsoft suite and the Visio software;
  • Excellent communication, interpersonal and coaching skills, and ability to act as a change agent;
  • Sound judgment accompanied by listening, analysis, synthesis, strategic thinking, and problem-solving skills;
  • Ability to create and lead complex work teams;
  • Demonstrated ability to work in a fast-paced environment;
  • Ability to build and maintain good work relations and partnerships with stakeholders and physicians in all sectors;
  • Proven ability to evaluate data using outcome measurement, develop work plans for the department and ensure follow-up;
  • Ability to travel within Vitalité Health Network's territory and the province;
  • Ability to work independently and as part of a team;
  • Physical ability to perform assigned work;
  • Good work history (performance and attendance);
  • Adherence to professional ethics principles, the Network's management philosophy and organizational values;
  • Adherence to Vitalité Health Network's confidentiality rules.
