We are searching for a Permanent, Full Time Social Worker to join our West Bedford Transitional Care Centre team based in Bedford, Nova Scotia.

The Social Worker is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.

The Social Workers focuses on relationships between people and their environments, which are integral to psychosocial, emotional, and spiritual wellbeing. Social Workers build upon the strengths of patients/families by focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.

The Social Worker supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.

About you

As the successful candidate, you have:

  • Bachelor of Social Work
  • Current registration with the Provincial Association of Social Workers
  • Minimum of two-years clinical experience
  • Communication requires motivating, persuading and negotiating, involves sensitive, complex or controversial issues using considerable probing, investigation and explanation to gain acceptance of ideas.
  • Asset:
    • Previous experience supporting admission and discharge of vulnerable populations with complex social situations; advising on care plans and implementing resources, giving the patient /family every opportunity to return to home/community.
    • Masters in Social Work

About the role

  • Completes an initial and on-going bio-psychosocial assessment to identify strengths and challenges that will influence optimal health outcomes and determine psycho-social risks for recurrent illness, disability, and health distress.
  • Determines appropriate therapeutic interventions to address the following, but not limited to: trauma, suicide and crisis, psycho-educational needs, addictions, grief and loss, resource needs, stress management, life altering transitions, mental health issues.
  • Develops and facilitates psycho-educational activities/groups for patients, and families to increase self-care and self-management.
  • Provides orientation to students, develops, and delivers presentations on pertinent topics and provides formal and informal peer mentorship.
  • Works collaboratively with the healthcare team to create a comprehensive plan for risk reduction and timely transition through assessment, planning, intervention, and evaluation.
  • Develops collaborative partnerships with community and government partners to identify and address systemic barriers enabling the patients, families, and stakeholders they work with to enhance their health and wellness.
  • As a member of the interdisciplinary care team, and based upon information gathered through the psychosocial assessment, the Social Worker will determine appropriate therapeutic interventions to address the following, but not limited to:
  • trauma
  • suicide and crisis
  • psycho-educational needs
  • addictions
  • grief and loss
  • resource needs
  • stress management
  • life altering transitions
  • mental health issues
  • Working collaboratively with the healthcare team, the Social Worker creates a comprehensive plan for risk reduction and timely transition through assessment, planning, intervention, and evaluation.
