Job Summary
The Registered Nurse IIC (Care Facilitator) is a Registered Nurse who functions in a leadership capacity, coordinating the daily activities of the units, including consulting and coordinating with other departments, units, and program areas, i.e. acute care, emergency departments regarding the more complicated health issues, complex admissions and readmissions. The Care Facilitator will be accountable for the continuity and coordination of resident care utilizing clinical judgment and critical thinking skills; assisting with the development, implementation and evaluation of resident quality and safety initiatives; and ensuring a high standard of resident care is maintained. The Care Facilitator will evaluate the effectiveness of resident care through such mechanisms as team conferences, ongoing feedback with team members/residents/family, and review of staff assignments. The Care Facilitator also participates as an active team member and acts as a liaison for team members in the interdisciplinary provision of care; facilitates and coordinates resident/family care with other disciplines, departments and programs; and performs other related duties as required.

Occasionally moves/lifts objects between 10 - 30 lbs, pushing files, charts and equipment.
Constantly sits to perform work on a computer to do research, write policies/procedures/manuals, prepare presentations, and read and prepare documents.
Regularly consults/interacts with other departments, units, and hospitals to coordinate timely patient/client/resident flow and to communicate regarding division/unit issues.
Regularly stands or walks to perform audits, give presentations, respond to patient/client/resident complaints and collaborate with the health care team to resolve issues.
Regularly performs fine finger/precision work on the computer or with machines that require accurate control and steadiness.
Visual concentration is required to view and report on documents, assess workplace situations, and/or to monitor the workplace regarding health, safety and risk management issues.
Exposure to emotionally distressed residents, family members and caregivers.
Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing or equivalent education or training is required.
Current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required.
Current BLS Certification is required.
Minimum of five (5) years' experience within the last seven (7) working in a geriatric setting with primary responsibility caring for patients/residents with chronic and/or complex needs is required. (Applicants are required to highlight relevant experience on their resume to be considered in the selection process.)
Speciality certification in gerontology is an asset.
Knowledge of the resident centered care philosophy.
Knowledge of long term care practices.
Knowledge of continuous quality improvement concepts.
Demonstrated resident centered care approach with residents and their families.
Demonstrated competency in organizational, leadership, interpersonal, team building, communication, teaching, conflict management and decision-making skills.
Demonstrated initiative and ability to work independently.
Demonstrated progressive leadership.
Demonstrated ability to develop and maintain successful working relationships.
Evidence of active involvement or willingness to participate in continuing professional education development.
Ability to facilitate staff and student clinical initiatives effectively.
Ability to work with residents in a fast-paced environment.
Membership in professional organizations/special interest groups is an asset.
A satisfactory record of work performance is required.
