Family Physician - Care of the Elderly, Temporary Full-Time (Approximately 12 Months)Family Medicine, Regional, Location to be Determined
Definition of work:
The physician will work to enhance the capacity and expertise in providing appropriate and effective care to the frail elderly in Central Health. This position provides an invaluable enhanced adjunct to key Central Health programs and works to address gaps in care surrounding the frail elderly and their healthcare needs.
The COE physician will facilitate collaborative participation in the Home First Integrated Network (HFIN) and Dementia Care Strategy to work with Nurse Practitioners and other members of the team to review comprehensive geriatric assessments, diagnoses, and provide recommendations on medication reviews.
It is envisioned that the COE physician will be actively engaged in program development activities to provide a medical specialist perspective in developing all aspects of the model of care for the elderly including a particular emphasis on further defining, refining and delineating the roles of the COE consultant and family physicians in the program model
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications:
* Eligible for registration and licensure as a Family Physician with the College of Physicians and Surgeons of Newfoundland and Labrador.
* College of Family Physicians of Canada Certificate of added competence (CAC) in Care of the Elderly.
* Satisfy credentialing requirements of Central Health.
* Work experience is an asset
* Consistent demonstration of Central Health's corporate values; Compassion, Accountability, Respect, Equity and Safety.
* A satisfactory record of work performance and attendance is required for this position.
* A satisfactory Certificate of Conduct from the RCMP or local police authority.
* Earned Medical Degree.
Hours of work: 75 hours bi-weekly; plus on-call obligations
Salary: $198,724-$215,097 annually
