Eagle Lake First Nation is seeking a compassionate and dedicated Community Health Nurse & Healthy Babies Healthy Children (HBHC) Worker to deliver culturally sensitive, community-based health services. This unique dual role supports both general community health needs and early childhood development through the HBHC program. The successful candidate will provide clinical nursing care, promote wellness and disease prevention, and support families from pregnancy through early childhood.
The position emphasizes a holistic and Indigenous-centered approach to health care, working in partnership with families, Elders, and other community members. Services will include home visits, immunizations, prenatal/postnatal education, chronic disease support, and coordination of parenting programs.
DUTIES AND RESPONSIBILITIES:
COMMUNITY HEALTH NURSING:
Conduct assessments, provide treatments, and manage follow-up care.
Deliver immunizations, screenings, and chronic disease support.
Promote health and wellness through workshops, outreach, and one-on-one education.
Assist clients with wound care, medication management, and other clinical needs.
Collaborate with public health agencies for disease control and prevention.
HBHC PROGRAM:
Provide support to expectant mothers, infants, and young children through home visits.
Deliver immunizations, screenings, and chronic disease support.
Guide families in nutrition, breastfeeding, and early childhood care.
Develop individualized family care plans and coordinate referrals to support services.
Facilitate parenting groups and knowledge-sharing workshops.
CULTURAL & COMMUNITY ENGAGEMENT:
Integrate traditional Indigenous practices and teachings into health services.
Work closely with Elders and community leaders to ensure culturally respectful care.
Build trusted relationships with community members to encourage participation.
DOCUMENTATION & PROFESSIONAL STANDARDS:
Maintain accurate and confidential client records.
Comply with health regulations and ethical standards.
Participate in program evaluations, training, and continuous improvement.
QUALIFICATIONS:
Registered Nurse (RN) licensed in Ontario, or Bachelor of Science in Nursing (BScN) or equivalent (preferred).
Minimum 2 years of experience in community, maternal, or public health nursing is an asset.
Experience working with Indigenous communities is an asset.
Certification in prenatal care, lactation, or early childhood development is preferred.
Valid driver's license and access to reliable transportation.
Current First Aid & CPR certification.
Must pass a Criminal Record Check and Vulnerable Sector Search.
