Position: Support Coordinator

EMPLOYMENT STATUS: 10 month contract, until March 31, 2026

Program Description:

The Emergency Treatment Fund (ETF) is a funding initiative offered by Health Canada to municipalities and Indigenous communities to address urgent substance use needs.

The HOME Team is a joint initiative between the Guelph Community Health Centre, the Sanguen Health Centre, and the Mount Forest Family Health Team, which will provide mobile wrap-around support services to individuals struggling with substance use who face barriers to accessing traditional services. The HOME team will expand on existing programs to operate two mobile outreach vehicles one in the City of Guelph, and one in Wellington County, and offer additional place-based supports.

Reporting to the Allied Health Supervisor, the ETF Support Coordinator will provide ongoing support to community members identified by Peer and Outreach workers on the HOME Team that have goals for which they are seeking support. The Support Coordinator will assist clients in pursuing their health and wellbeing goals by coaching, advocating, liaising with other supports, and providing system navigation.

Knowledge, Skills and Abilities:

  • Post-secondary degree or diploma in social service-related discipline.
  • Emergency First Aid and CPR Certification Level C Mandatory.
  • Drivers license and insurance Mandatory
  • Experience working with individuals marginalized by their mental and/or substance use health, alienated from services, and frequently in crisis, both in-patient and community settings.
  • Demonstrated knowledge of community resources related to the served population in Guelph.
  • Awareness of and comfort attending community locations such as social service offices, community food pantries, drop-in centres, and outdoor spaces frequented by relevant clientele
  • Ability to work independently, in the community, and within an interdisciplinary team environment
  • Engaging, collaborative and creative, astute to clients' needs with the ability to adapt and be flexible.
  • Excellent assessment, communication and organizational skills.

Principal Job Duties:

  • Collaborate with the client and their supports to develop an individualized care plan.
  • Coach clients to define goals, advocate for themselves to relevant services, and foster self-agency
  • Act as an advocate on behalf of clients by providing support accessing food, housing, financial assistance, as well as mental health and substance use services
  • Maintain client records, assessment notes, and correspondence within the safeguard of confidentiality
  • Receive and support referrals from clients, service organizations, and relevant institutions
  • Coordinate with other outreach-based providers and organizations to provide responsive, community-based care safely
