Job Description

Reporting to the Clinical Services Manager, you are responsible for the day-to-day management of the department. You will be responsible for promoting excellence in patient and family care by supporting integration of best practice, promoting ongoing learning and development of the nursing team, fostering inter-professional communication and relationships, and ensuring that patients receive the right care in the right place at the right time.

Accountabilities:

  • Develop and implement program improvement and quality initiatives related to nursing care with utmost diligence
  • complete or support completion of daily assignments and acuity scales.
  • Coordinate with the rehabilitation admission coordinator, access and flow coordinator, and Admitting Department to ensure appropriate bed assignments. Liaise with other members of the inter-professional team to facilitate a coordinated approach to patient discharge and transfer
  • Conduct and oversee Inter-professional discharge rounds twice weekly
  • Manage and lead thrice-weekly bullet rounds
  • Collaborate strategically with the inter-professional team, including Home and Community Care to optimize available resources, promoting efficient patient flow and ensuring a positive patient experience
  • Establish and maintain professional relationships with physician team members
  • Maintain comprehensive knowledge of emergency preparedness protocols and procedures, ensuring appropriate responses to emergency codes
  • Coordinate with facilities management and/or the access and flow coordinator to ensure timely and efficient bed/room cleaning and portering service
  • Work with FIM/NRS coordinator to ensure FIM completion within time timeframe for submission as per protocol
  • Working knowledge of FIM/NRS systems
Qualifications
  • Currently registered and in good standing with the College of Nurses of Ontario
  • Rehabilitation Nursing Certified (CNA)
  • Minimum three years recent experience practicing as a Registered Nurse
  • Recent rehab experience required with exemplary clinical skills
  • Functional independence measure training required
  • BScN preferred
  • Understanding of the issues related to rehabilitative nursing as they apply to patient, families and the multidisciplinary team
  • Excellent English language skills (reading, writing, comprehension and speaking)
  • Current BCLS
  • Previous leadership experience within the past two years would be an asset
  • Excellent communication and interpersonal skills
  • Excellent organizational skills
  • Excellent critical thinking and problem-solving abilities
  • Demonstrated ability to be flexible to work in a frequently changing environment
  • Demonstrated leadership skills, good conflict management and negotiation abilities
  • Demonstrated ability to lead and function as a member of a multidisciplinary team
  • Demonstrated commitment to continuing education
  • Excellent attendance, punctuality and work record
  • Membership in professional organization and/or related interest groups preferred
  • May be required to travel between and work at all Osler sites

Additional Information

Hours: Currently Days, 8 hour shifts (subject to change in accordance with operational requirements)

Salary:
Minimum: $43.19 per hour
Maximum: $60.12 per hour
