We're currently looking for a Care Manager to oversee care operations at both of our beautiful long-term care homes located in Abbotsford, BC.
The Opportunity: Care Manager (Full-Time | Two Sites: The Mayfair & The Oxford)As a Care Manager, you'll work closely with the Director of Care to lead, support, and empower the care team. You'll play a vital role in ensuring person-centered care, maintaining high standards of clinical documentation, and promoting safety and quality across both sites.
What You'll Be Doing:- Supporting and onboarding new care team members, ensuring all licensing, records, and contracts are up-to-date
- Conducting meaningful performance reviews and addressing performance or attendance issues
- Overseeing infection prevention and control measures, including enhanced protocols during outbreaks
- Managing care-related audits, including ADLs/Kardex reviews, delivery of care, and cleanliness inspections
- Handling incident follow-ups, coordinating safety huddles, and maintaining communication with families
- Supporting quality improvement initiatives such as falls prevention and restraint reduction
- Ordering and managing care supplies and maintaining care-related budgets
- Leading monthly OHS and care staff meetings, preparing and posting meeting minutes
- Providing ongoing education and training for care aides and nurses on topics such as wound care, dining experience, and violence prevention
- A Licensed Practical Nurse (LPN) in good standing with the BCCNM
- Minimum 2 years of experience in a supervisory or leadership role in long-term care
- Strong understanding of regulatory requirements in long-term care settings
- Excellent communication, organization, and leadership skills
- Confidence using electronic medical records and conducting audits
- A collaborative mindset and a passion for resident-centered care
