We are seeking PSWs for a community client in Port Severn. Proposed shift is 1pm-5pm, 3-4 days per week. The client is looking for care 17 hours per week. The ideal candidate is able to commit to this schedule on an ongoing basis. There is potential to pick up extra shifts with other clients in the area as well.

The successful candidate's areas of responsibility will include, but are not limited to, the following:
- Assisting with Activities of Daily Living (ADLs) and personal care including eating, dressing, transferring, toileting, hygiene, grooming, and bathing as per client's care plan
- Driving client to physiotherapy appointments on a regular basis
- Supporting client with assigned physiotherapy exercises daily
- Preparing meals for client and completing light housekeeping as needed
- Providing medication support and management
- Providing cognitive, social, emotional, and spiritual support
- Maintaining open communication with client and their family
- Monitoring care plan and adding detailed documentation with any changes and updates
- Responding to all emergency situations
- All other duties, as assigned
Key qualifications for this position will include, but are not limited to:
- PSW certification from a public college/learning centre OR additional NACC certification is **required**
- Previous working experience as a community PSW is **required**
- Access to a reliable vehicle and a valid drivers license
- Valid CPR/First Aid certification
- Completion of a Police Vulnerable Sector Check
- Satisfactory professional references
- Ability to accurately follow written and verbal instructions
- Must be able to anticipate problems and use sound judgment and tact in handling confidential matters
