Grandview Medical Centre Family Health Team is seeking an 18-month contract MSW Social Worker to join our progressive Family Health Team. This position will be a minimum of 3 days/week (Part-time) to a maximum of 5 days/week (Full-time)

Grandview Medical Centre FHT is a busy team with 16 physicians and multiple allied health professionals working collaboratively in Cambridge, Ontario. The Social Worker would be working in a primary health care setting as part of the Mental Health team.

The successful candidate would possess the following:

• Required experience working with children and youth
• Successful completion of a masters degree in social work (M.S.W.)
• Membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
• Proof of Liability insurance policy required
• 2+ years clinical experience providing psychosocial assessments, counselling and psycho-educational interventions
• Demonstrated knowledge in trauma-informed care
• Experience using evidence-based approach in short term counselling setting
• Excellent interpersonal, verbal and written communication skills
• Excellent organizational, time management and problem-solving skills
• Experience working with patients and families in a multi-disciplinary team within a primary care setting is an asset

4 weeks vacation (prorated based on number of days per week)

Up to 5 days paid for Continuing Medical Education (CME)

Up to $750 for Continuing Medical Education (CME)
