Salary: $45 - $50 hour

Reporting to the Clinical Lead - RN, the Public Health Nurse/Home Care Nurse (RN) will conduct therapeutic interventions, administer care and provide health advice to members within practice guidelines and other acts and legislation that govern practice. This position is front facing for the akisqnuk First Nation Health Center and will act as the community health expert. This position will apply the knowledge of Social Determinants of Health specific to First Nations Communities as well as a bi-cultural lens to work activities.

NOTE: this position is casual/on-call without a guarantee of weekly hours.

Duties: Clinical Practice

  • Provide direct patient care including assessments, triage, pain management, and care planning.
  • Deliver preventative care, early screenings, prenatal/postnatal services, and chronic disease management.
  • Offer interventions based on medical directives, monitor communicable diseases, and ensure infection control.
  • Conduct community health and needs assessments; organize family health clinics and support services.
  • Collaborate with primary care providers and partner agencies to coordinate patient care.
  • Offer peer counseling, referrals, and follow-ups.

Nursing Leadership & Operations

  • Ensure client safety through home assessments and risk mitigation.
  • Lead staff development through training, coaching, and performance evaluations.
  • Implement quality assurance protocols and maintain high standards of care.
  • Manage challenging client or staff situations with professionalism and resolution skills.
  • Foster a collaborative team environment and promote patient-centered care.
  • Oversee confidentiality, documentation, and compliance through regular audits and inspections.

Home & Community Health

  • Deliver culturally sensitive care across diverse populations.
  • Provide education on equipment use, medication management, and lifestyle changes.
  • Support patients with chronic illnesses through personalized care plans and regular follow-ups.
  • Administer immunizations and manage COVID-19 response efforts including phone assessments.
  • Coordinate care through EMR documentation and communication with the interdisciplinary team.

Collaborative Care & Relationships

  • Work within multidisciplinary teams to develop and execute individualized care plans.
  • Advocate for patient rights and ensure care aligns with their preferences and values.
  • Facilitate smooth care transitions between settings, reducing hospital readmissions.
  • Maintain strong relationships with primary care and community providers to optimize patient outcomes.

Qualifications:

  • Bachelor of Science in Nursing
  • Minimum of 5 years of nursing experience

This position is casual without a weekly guarantee of hours.
