- Provide direct nursing care, including medication administration, wound care, and other medical procedures as required.
- Assist with tube feeding, catheter care, and respiratory support as needed.
- Support activities of daily living (ADLs), including personal hygiene, grooming, and mobility assistance.
- Conduct health assessments and monitor changes in the client's condition, reporting any concerns to the appropriate healthcare team members.
- Coordinate and communicate with families, caregivers, and healthcare providers to ensure holistic care.
- Maintain accurate documentation and charting of client care and services provided.
- Provide emotional support and companionship to clients and their families.
- Follow and adhere to all health and safety guidelines, infection control protocols, and best practices in home health care.
- Active LPN License in good standing with the BC College of Nurses and Midwives (BCCNM).
- CPR and First Aid Certification (current and valid).
- Experience in senior care, home care, or community care is an asset.
- Strong understanding of healthcare protocols, infection control, and personal hygiene practices.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work both independently and as part of a care team.
- Physical ability to perform the duties of the position, including lifting, transferring, and assisting clients with mobility.
- A valid driver's license and reliable transportation is an asset but not required.
