Duties & Responsibilities (Overview)

• Facilitate youth intakes and discharges
• Share information, communicate concerns, coordinate care with internal/external resources
• Develop, implement, oversee, review, update & communicate youth case plans
• Identify and bridge support/resource gaps
• Advocate for youth (i.e education, family, medical, mental health, substance use)
• Safety planning with youth
• Oversee the maintenance and upkeep of the peer home
• Crisis Intervention/management, rotating crisis line coverage (1 week on 5 weeks off)
• Facilitate and attend program/team meetings as required
• Comply with agency, program, and funder reporting requirements
• Ensure compliance with current agency policies, procedures, guidelines, and best practices
