Bilingual Intake Worker

Full Time Indeterminate position

(35 hours per week)

South-East Ottawa Community Health Centre is a community health centre, offering a variety of health and social services to residents living in the South-East Ottawa Community Health Centre catchment area. We require a bilingual (French/English) Intake Worker to work with our Social Services team.

JOB SUMMARY

Using a holistic, client-centred approach, the Intake Worker (IW) provides practical community-based support, information & referral, advocacy, mental health assessment, short-term counselling, harm reduction services, system navigation, and episodic wrap-around supports. The IW needs to think creatively to support high-needs clients with limited resources. This position will serve as a social-prescribing point person within the team. In addition, this person acts as a resource person and liaison between intake and other programs in and outside the center that could be of benefit to the client.

QUALIFICATIONS:

BSW or equivalent with at least 3 years related experience with counselling/intake and poverty issues.

Take initiative to support clients, problem solve and propose supports and solutions.

Experience working with vulnerable communities.

Excellent skills in, assessment, advocacy, outreach & conflict resolution.

Excellent knowledge of community resources and the determinants of health.

Ability to work independently and with members of an interdisciplinary team.

Excellent time management and organizational skills

Excellent verbal and written communication skills with fluency in English and French; other languages an asset.

Registered with the Ontario College of Social Workers and Social Service Workers (or eligible for registration).

Solid computer skills and experience using a client database.

Ability to contribute to the larger South-East Ottawa team.

Ability to work evenings & weekends, onsite in-community & remote work required.

Adaptable to a changing environment.

Must show proof of double vaccination for Covid-19.

Recent Police Record Check for the Vulnerable Sector is required.

RESPONSABILITIES

Develops and co-ordinates effective support and integration of social services that address the needs of equity-seeking groups.

Combination of working in community (clinic), onsite and virtually.

Provides mental health supports: including completing mental health assessments, counselling and psychosocial support and resource.

Assists clients to access support networks and acquire necessary skills to establish themselves and become productive community members.

Participate in and share educational opportunities and resource materials related to social prescribing.

Resource person for social prescribing at the centre.

Identify a social prescribing framework for the centre: outcomes, asset maps etc.

Maintains linkages and collaborative working relationship with other community and social services agencies.

Completes required paperwork, and maintains client records, program statistics and schedule.

Acts as an Ambassador for SEO and equity seeking groups in the SEO community.
