POSITION FUNCTION:
The Outreach Case Manager provides wraparound case management services to refugee and immigrant youths and adults. The Outreach Case Manager provides comprehensive case planning, outreach, and support to clients coping with multiple barriers to their settlement. The Case Manager will help empower refugee/immigrant youth towards the goals of greater independence and self-sufficiency.
KEY DUTIES AND RESPONSIBILITIES:
Perform outreach to high-needs refugees and immigrants and determine client eligibility
Conduct a comprehensive needs/strengths assessment for each client
Refer ineligible clients
Develop, revise, and support client action plans that set goals to achieve during the service period
Assist and facilitate clients in implementing their action plan
Plan, organize, and conduct settlement and life skills orientation sessions one-on-one or in groups
Plan, organize, and conduct groups and activities for social and emotional support if necessary
Provide continuous case monitoring and follow-up, and prepare clients exiting the program
Organize and participate in client case-conferencing with participating organizations
Provide settlement counselling and practical assistance to clients
Link and refer clients to various services and provide ongoing liaison support
Perform home visitation, community outreach, accompaniment, and supported access to external services through cultural brokerage
Update and maintain accurate, up-to-date case files, case notes, and case records; document service and counselling activities daily, both electronically and in hard copy, using the prescribed format
Collect statistical information and produce or assist in the preparation of reports to meet contractual requirements
Keep the program coordinator and/or manager informed of the achievement of outputs and outcomes.
Contribute to the development of program planning, facilitation of group work (life skills orientation, recreational outings, and emotional/social support groups for clients)
Administer and support volunteers in support of group activities if necessary
Collaborate with partner providers to deliver a seamless service experience for clients
Participate in marketing activities as required
Provide back-up for designated staff as required
Participate in supervision, departmental, all-staff, and relevant community meetings, in-service training, and other training
QUALIFICATIONS:
A Bachelor's degree in a relevant field, or high school education and a certificate diploma in an appropriate area, such as community social services, community development, or equivalent experience
A valid and satisfactory criminal record check is required
Language Competence: In addition to the English language requirements (English language proficiency at a minimum of North American Grade 10 or assessed CLB level 6 for reading, writing, speaking, and listening or equivalent TOEFL or IELTS scores), candidates must also be able to speak Spanish as a second language.
Strong working knowledge of settlement integration issues facing vulnerable immigrant and refugee youth, young adults
Solid understanding of mental health issues affecting vulnerable immigrant and refugee youth, youth, young adults, adults, and their families
Knowledgeable about local community resources, professional and specialized services for at-risk or marginalized populations, and how to use those resources for the benefit of the clients
Good Knowledge of BC Systems and the capacity to connect clients to mainstream services
Experience in cross-cultural communication and service methods appropriate to clients with low literacy and low English skills
Experience working with clients suffering from social, emotional, and mental health issues
Experience and a strong understanding of case planning and case management principles and techniques (specific to staff involved in case management)
Experience in teaching Life Skills, Literacy, and Essential Skills (specific to staff delivering skills development services)
Experience with Youth and program planning, organizing recreational activities and social events
Training in facilitation and grassroots support groups
Proficiency in computer equipment and ability to use Microsoft Office tools
Ability to work in a team with internal and external stakeholders
COMPENSATION: $28.58 - $29.25 per hour
HOURS:
35 Hours per week, some evenings and weekends may be required.
The successful candidate may be permitted to work a hybrid work model (combination of in-office and remote work).
The successful candidate must be able to commute to the Highgate location at 310 & 315 - 7155 Kingsway, Burnaby.
BENEFITS:
A competitive benefits package is provided with contracts of at least 1 year in length and which offer a minimum of 17.5 hours of work per week.
